A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.

Observation. Upon walk through the facility licensing observed paint chipping in a few areas on the walls of the facility.

Corrective action. Ensure the facility is in good repair.

A licensee must keep, for each child, a record showing the following information: (a) name, sex, date of birth, medical insurance plan number and immunization status.

Observation. Licensing reviewed 6 children's files and one child's file was missing an MSP number.

Corrective action. Ensure children's files are complete.

A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (a) a criminal record check for the person.

Observation. Criminal Record Check was expired for one person.

Corrective action. Ensure CRC is valid.

A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (b) character references in respect of the person.

Observation. A minimum of three character references were not obtained for the manager. This is a repeat contravention from 2024.

Corrective action. Manager said they will add two more references to their file.

A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.

Observation. during the inspection of the outdoor facility licensing noticed the following in regards with the fence surrounding the facility: - the last panel of the fence (behind the shed) had a missing plank at the bottom of the fence and the bottom of the fence was rotted and had some holes in it. - several panels of the fence have been patched up because the fence has holes in it. - when licensing pushed on the fence to verify the sturdiness of the fence the fence moved. The fence will have to be replaced or repaired.

Corrective action. Licensee will look into getting some fencing quotes and looking into replacing the fence of the entire facility.

A licensee must keep, for each child, a record showing the following information: (a) name, sex, date of birth, medical insurance plan number and immunization status.

Observation. Licensing reviewed 6 children's files and noted that one child's file was missing a record of immunization.

Corrective action. Licensee said she will follow up with the parents of the child in regards to immunization.

A licensee must keep, for each child, a record showing the following information: (d) name and telephone number of a parent, medical practitioner and emergency contact.

Observation. Licensing reviewed 6 children's files and 3 children's files were missing an emergency contact.

Corrective action. Licensee said that she will follow up with the parents in regards to the emergency contact.

A licensee must not employ a person in a community care facility as (a) an educator or an assistant unless the person holds a certificate issued under Division 2 [Employee Qualifications], which must be verified by the system maintained by the early childhood educator registry. (Educator or assistant certificate validation)

Observation. Licensing reviewed 3 staff files and noted that the validation of ECE and ECEA certificate has not been completed.

Corrective action. Ensure all staff files are complete.

A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (b) character references in respect of the person.

Observation. Licensing reviewed three staff members files and noted that a minimum of two character references were not obtained.

Corrective action. Ensure all staff files are complete.

A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (f) evidence that the person has complied with the Province's immunization and tuberculosis control programs.

Observation. Licensing reviewed 3 staff files and noted that immunization status was not obtained for one or more employees.

Corrective action. Ensure staff files are complete.

A licensee must keep, for each child, a record showing the following information: (d) name and telephone number of a parent, medical practitioner and emergency contact.

Observation. During the routine inspection 7 children's files were reviewed and two children's files were missing a doctor's name. This is a repeat contravention from 2022.

Corrective action. Please ensure all children's files are complete.

A licensee must keep, for each child, a record showing the following information: (a) name, sex, date of birth, medical insurance plan number and immunization status.

Observation. During the routine inspection 7 children's files were reviewed and one child's file was missing a record of immunization.

A licensee must ensure that children do not have access to any object or substance that may be hazardous to the health or safety of a child. Observation (CORRECTED DURING INSPECTION): -An unsecured electrical cord accessible to children where children play. This was placed out of reach of children during inspection. -One electrical outlet in the bathroom and one electrical outlet in the children's play area were uncovered. Covers were replaced during inspection

Corrective action. Please ensure that children do not have access to any object or substance that may be hazardous to the health and safety of a child.

Observation. Licensing reviewed five children's files, two files did not contain the name of the medical practitioner, and three did not contain the telephone number of the medical practitioner, and one file did not contain an emergency contact.

Corrective action. Please ensure to keep for each child a record showing the medical practitioner's name and telephone number, and an emergency contact.

A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees. Observation (CORRECTED DURING INSPECTION): Garbage bags used to darken the nap room window, accessible to children could pose a suffocation risk. This was corrected on site.

Corrective action. Please ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.

A licensee must keep, for each child, a record showing the following information: (c) daily attendance record, indicating for each day whether the child is absent or, if the child is present, the time of arrival and departure. Observation (CORRECTED DURING INSPECTION): One of the six children in attendance had no record of attendance and time of arrival. Licensee stated that the child was a drop-in child, therefore not signed-in.

Corrective action. Please ensure all children have record showing the following information: (c) daily attendance record, indicating for each day whether the child is absent or, if the child is present, the time of arrival and departure.

Observation. -The wall mounted towel rod in the bathroom was removed from the wall giving children access to the drywall. -The door mat had a hole in it where below there is as hole for draining rain water. This could pose a tripping hazard.

Corrective action. Please ensure the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.

A licensee must keep current records for each child showing (a) the information set out in subsection (2), (b) if applicable, the information and agreements set out in subsections (2.1) and (2.2), and (c) the consents referred to in subsection (3).

Observation. 2 children's records were unavailable for review. 2 children had photographs that were not current and would not readily identify the child in an emergency.

Corrective action. ensure that each child has a current records with the information set out in subsection 2.
