  Observation. A care plan for a child who requires extra support was not available at time of inspection. Child records must be kept in a single place within the facility at all times so staff can immediately access the information contained in those records. Submit to licensing by January 16, 2026, evidence that the care plan is in the child's file.

• C3.1 - Does the facility have sufficient employees who possess adequate training and experience to meet the care, supervision, and needs of the children?Non-critical

  Observation. One staff file is missing immunization status. Unknown staff immunization status may put children at risk if a disease outbreak were to occur. Submit to licensing by January 16, 2026, evidence the missing staff record has been completed and is in the staff file.
