    Observation. Licensing observed the outdoor area had a little play house with exposed nail that was sharp and accessible to the children, two metal chairs that were rusty with sharp chipping paint. One of the slides, being used by children at the time of inspection, with an exposed screw.

    Corrective action. Ensure equipment and furniture are sturdy, safe, clean and free from hazards.

    Observation. Licensing audited eight staff files. Two staff files had references listed but not date, notes or otherwise indicating that the references had been obtained.

    Corrective action. Ensure all staff files are complete, including character references.

    Observation. Licensing audited eight staff files. Two staff files had expired verifications from 2025 and two staff files did not have verification completed.

    Corrective action. Ensure all staff files are complete and have had the certificates verified.

    Observation. Regarding intake February 2, 2026. Through the investigation process, Licensing determined that [child A] was missing or wandering, and it was not reported to the Medical Health Officer within 24 hours as required.

    Corrective action. Ensure the Medical Health Officer is notified of when a child is involved in, or may have been involved in a reportable incident as described in Schedule H. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: On March 18, 2026, Licensee submitted a corrective action plan to Licensing. The corrective action plan included: a missing and wandering child policy was created and implemented. The policy included clear procedures to follow and included staff must immediately notify the Manager [or delegate], conduct a search and if the child was not located, the facility must contact 911 and the child's parent. The Manager [or delegate] was responsible for notifying the Medical Health Officer within 24 hours of occurrence. Staff signed an acknowledgement form of the policy and it would be added to the staff file. The corrective action plan was reviewed and accepted by Licensing.

    Observation. Regarding intake December 1, 2025, through the investigation process, Licensing determined that the facility had a policy requiring staff to communicate clearly and set limits with children in a positive manner. Licensing found that a staff member did not set limits in a positive way. The staff member self-reported that they used a 'bear hug' to move a child as a behavioural guidance technique. Using a 'bear hug' to move a child is not consistent with positive guidance or the facility's policy.

    Corrective action. Ensure that behavioural guidance is appropriate to the age and development of the child. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: Between February 17 and 23, 2025, Licensee submitted a corrective action plan to Licensing. The corrective action plan included: The behaviour guidance policy would be updated to include guidance strategies specific to each age group. Staff would be required to attend mandatory training on the updated policy and provide written acknowledgement that they have reviewed and understood it. The training would be completed by March 24, 2026. The corrective action plan was reviewed and accepted by Licensing.

    Observation. Licensing observed heavy items stored in the Group Child Care (Under 36 Months) in the bathroom, cubby area, nap room and kitchen. In the Group Child Care (30 Months to School Age), there were heavy items stored on top of shelves in three rooms and two shelves, measuring over four feet were not secured to the wall. In the outdoor play area, there were blackberry bushes and holly accessible to the children.

    Corrective action. Ensure that children do not have access to any object or substance that may be hazardous to the health or safety of a child.

    Observation. Licensing audited nine children's files. Six children's files did not have a record of any person not permitted access.

    Corrective action. Ensure a record of any person who is not permitted access is kept on file.

    Observation. Licensing observed the attendance sheet did not reflect the number of children in attendance this date. Three children were not signed in.

    Corrective action. Ensure a daily attendance record, indicating for each day whether the child is absent or, if the child is present, the time of arrival and departure.

    Observation. Licensing observed several medication forms did not have a time and date for administering Tylenol or Advil for teething.

    Corrective action. Ensure medication is administered to the child in the amount and at the times specified by the child's parent or in the child's record or care plan.

    Observation. Entanglement test device did not pass on the large side of the double side of the Little Tykes double slide. The above noted item represents an immediate risk to the health and safety of children in care and must be corrected immediately. The manager removed the slide this date.

    Corrective action. Ensure equipment passes the entanglement test device.
