  Observation. Licensing observed that the Early Childhood Educator Assistant did not have a verified certification.

  Observation. Licensing audited attendace records for the week of September 4, 2024 and noted blank spaces that did not clearly indicated if a child was absent or present. Corrected at time of inspection.

  Observation. Licensing observed that a file containing records for a employee who worked at the facility in August 2024 were kept offsite. Manager stated the file was kept a at another location.

  Corrective action. Ensure records are kept in a single place at the community care facility.

  Observation. Licensing observed that a log of inspections, maintenance and repairs had not been kept since September 4, 2024. This item is carried over from Routine Inspection dated March 27, 2024.

  Observation. Licensing audited three staff files, one was missing character references for a person employed.

  Observation. Licensing audited attendance records for the week of February 1, 2024 and noted sign out times missing for children in care on February 2, 5 and 6, black spaces were also noted on the attendance records that did not clearly indicate if a child was absent or present

  Corrective action. Ensure attendance record show time of arrival and departure and if a child is absent or present.

  Observation. Licensing observed that no care plan had been created for a child who required extra support at the facility. The child was attending on this day and had a staff working as a one on one Support Worker.

  Observation. Licensing observed that a Criminal Record Check for a staff and evidence of training for a Responsible Adult were not kept at the facility. This was corrected at time of inspection.

• A licensee must keep current records of each of the following: (f) a log of minor accidents, illnesses and unexpected events involving children, that did not require medical attention and were not reportable incidents described in Schedule H.

  Observation. Licensing observed that the last minor incident recorded was noted as 2015. Manager started she was not aware minor incidents needed recording and a record had not been kept.

  Corrective action. Ensure a record is kept for minor incidents, illnesses and unexpected events.

• The licensee must review the care plan at least once each year with a parent of the child requiring extra support and any person requested by the parent.

  Observation. Licensing audited eight children's files. Two children's files were missing the doctor's name and number. Manager stated the families use a walk in clinic.

  Corrective action. Ensure the name and number for a medical practitioner is obtained.

  Observation. Licensing observed one care plan that was last reviewed January 19, 2021.

  Corrective action. Ensure a care plan is reviewed at least once per year.

• A licensee must keep, for each child, a record showing the following information: (d) name and telephone number of a parent, medical practitioner (doctor's name and number does not apply to REC only) and emergency contact

  Observation. Licensing audited eight children's files. Two children's files were missing the doctor's name and number. Manager stated the families use a walk in clinic.

  Corrective action. Ensure the name and number for a medical practitioner is obtained.
