  Observation. Licenisng audited nine children's files. One file was missing emergency contact. The parent was listed as the emergency contact and no other names were obtained.

  Corrective action. Ensure all children's files have record of emergency contact.

  Observation. Licensing audited eight staff files and a ninth staff did not have a staff file this date that was working in ratio in the Gym 1 program at the time of inspection. The staff stated she did not have a staff file.

  Corrective action. Ensure Licensee has records kept in a single place for sections 56-58 the entire time the staff is working.
