    Observation. The following items have been outstanding since May 16, 2024. These items were still observed to be in non compliance and are safety hazards: 1) On July 8, 2025, Licensing observed heavy loose slate pavers in the corner of the yard (at the bottom of the slide against the building). Some pavers were standing against the building. The lead staff informed licensing the pavers had been a pathway through vegetation. 2) Licensing observed multiple area with rotten and loose wood stumps and wood log barriers. Some are in worse shape than others, i.e. jagged edges and crumbling. For example the upright stumps by the sheds, wood steps to the upper level, wooden logs around the sand pit. • Facility Management is currently obtaining quotes to begin the necessary work. • The Management has been inform that design to be submitted to CCFL for review and approval prior to installation or construction.

The lead staff informed licensing the pavers had been a pathway through vegetation. 2) Licensing observed rotten and loose wood stumps and wood log barriers. Some are in worse shape than others, i.e. jagged edges and crumbling. For example the upright stumps by the sheds, wood steps to the upper level, wooden logs around the sand pit. The lead staff informed licensing that two recent yard walk through's took place with YMCA's management (including maintenance management) and hospital management to address the non compliance outstanding since May 16, 2024. - On July 21, 2025 the manager e-mailed the following plan of action: • On July 15, 2025 the YMCA Facilities Manager and their supervisor visited DM to review all playground-related NIC items. • Facility Management is currently obtaining quotes to begin the necessary work. • The current manager is working with the team to develop a transition plan so the incoming manager can continue this project and ensure compliance. - On November 4, 2025, an email exchange between the YMCA Property Manager and the landlord authority correspondence that they are in the process of seeking clarity on the allocation of responsibilities and the associated costs related to addressing the repairs.

    Medications that staff have agreed to administer are readily accessible to staff and are administered in the correct dosage at the correct times. [CCLR Sec. 53(1)]

    Observation. Licensing observed an Anaphylaxis care plan with the following information: "Reactin only for mild skin rash 2.5 ml" "Ventolin for wheezing" "EpiPen for any allergic reaction more than skin rash" "At least 2 symptoms and 2 organs involved" The above directions are confusing and not clear. Vancouver Coastal Health does not support the use of Antihistamines such as Reactin for treating an Anaphylaxis reaction. Epinephrine is the first line of medication which should be used for the emergency management of a person having a potentially life threatening allergic reaction. Antihistamines (e.g. Reactin) and asthma medications should not be used instead of epinephrine for treating anaphylaxis. The Reactin had expired in September 2024 and the EpiPen had expired in December 2024.

    Observation. Licensing observed the following: 1) Torn vinyl on the back of the housekeeping area cabinets (leading to the outdoor yard). 2) Loose and broken off vinyl on the lower kitchen cabinets located across the sink. 3) Carpet with curled up comers under the water table that posed a tripping hazard.

    Observation. FOM stated they would address emergency / safety items including the removal of exposed nails and screws. They stated staff would have to alert FOM when these were detected. While some of the exposed nails have been removed by staff and FOM already, there were more observed today. Staff will continue to monitor as part of their daily inspections and if they cannot remove, they will contact FOM to do so. The latch between the toddler and 3-5 yard has been secured. FOM stated they will replace the missing panel on the exterior of the building in the northeast corner. FOM took photo. FOM stated they will replace the missing panel on the sand hut. FOM took photo. YMCA has replenished sand. A new issue today was the pooling of water at the top of the slide. FOM stated they will address this and took photo. FOM will fill recessed area that pools with water with gravel. FOM was clear that their contract does not cover removing and or replacing wood stumps & retaining logs that require attention as per routine inspection of May 16. FOM stated the YMCA will have to find a contractor for this work. CCFL made clear that these actions will have to be addressed whether by the YMCA or FOM.

    Observation. Staff team stated they have reported yard maintenance issues to the hospital facilities team for follow up. During this inspection, facilities personnel visited the childcare and took photos of areas in need of attention. The childcare was licensed in 2014 and a number of the exposed wood elements have aged out, in particular many of the wood stump features throughout the yard. CCLOs reviewed the status of the wood elements with staff team member, noting areas in need of attention. Staff team noted that some of the wood panelling is missing on the building and hut. Top step on the steps near the slide must be monitored over the next year (a sizable crack has formed that may grow). All staff stated that sand was on order for each yard and would be replenished soon. CCFL confirms this is required. Discussed fibar requirements - depth in relation to the height of structure and function - with staff team member.

    Observation. In the gross motor room Licensing observed that there was a limited selection of toys and equipment for 6-8 children. Licensing observed some wooden blocks, some dishes, a doctor's kit and some books. This is not an adequate amount of activities, toys and equipment.
