Observation. Licensing observed that the vinyl cover on all nap mats was ripped exposing the porous material underneath. Nap mats are no longer in good repair and cannot be properly sanitized. Blankets to cover the nap mats were stored on top of each other in a crib, touching each other. This poses a risk of cross contamination.

Observation. Licensing observed a shelf in one of the additional play rooms carrying program supplies. The shelf was observed to be over 4 feet in height and poses as a tipping hazard.
