  Observation. The outdoor check list is not being done by the staff daily. The staff stated that are checking the playground,however no written daily records was available.

  Observation. Licensing observed that one of the folding picnic tables was no longer in good repair condition. The table's bench was ripped and peeling, creating rough edges that pose a potential hazard to children using the area.
