  The facility, the furniture, equipment, fixtures and supplies are of sturdy and safe construction.  They are easy to clean, and are clean and in good repair and free from hazards. [CCLR Sec. 13(2) and Sec. 15(1)(a)]

  Observation. Licensing observed the following: - Various areas of the facility are showing signs of wear and build-up of grime. Affected areas include facility walls, children’s play equipment and toys, children’s couches with flaking surfaces, ripped wallpaper, modular climbing shapes shedding their exterior material, and carpets with visible stains. These conditions indicate that the areas are either not clean, cannot be effectively cleaned, and/or are not in good repair. - Sliding closet doors in the shared hallway rock back and forth when used, which may cause a door to come off its track over time. - Gate leading to sandbox in the outdoor area is made of wood and deteriorating along the top edge, exposing screws.

  Observation. Licensing observed electrical cords hanging loosely in both nap rooms.

  Observation. The Licensing Officer observed that the playground shows signs of wear and tear, with surfaces that are no longer considered safe for the age group of infants and toddlers. Several wooden materials and features were in a state of disrepair, including rotten logs with rough edges. The protective ground surface was observed to be compromised, with tree roots growing above ground and creating uneven surfaces that pose a tripping and falling hazard.
