A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.

Observation. A long wire of decoration lights was loosely put around the play structure in main play room that was accessible to children.

Corrective action. Please ensure that a healthy and safe environment is provided to children.

A licensee must ensure that children do not have access to any object or substance that may be hazardous to the health or safety of a child.

Observation. Toilet bowl cleaner and cleaning supplies accessible in the children washroom. A projector wire was on the floor and not secured.

Corrective action. Please ensure that children do not have access to anything that may be hazardous to their health and safety.

A licensee must keep, for each child, a record showing the following information: (d) name and telephone number of a parent, medical practitioner and emergency contact.

Observation. Review of 6 children files observed one child was missing doctor's name and phone number on file.

Corrective action. Please ensure that all children files are complete. n

A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (a) a criminal record check for the person.

Observation. Criminal records clearance was not site specific on one staff file.

Corrective action. Please ensure that staff files are complete and up to date.

A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.

Observation. The following were observed during inspection; One shelf in small play room needs to be anchored; Small carpet was curled up and posing tripping hazard.

Corrective action. Please ensure that a healthy and safe environment is provided to all children while in attendance

A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (a) a criminal record check for the person.

Observation. Criminal Record Check was expired in March 2023 for substitute staff.

Corrective action. Ensure all staff files are complete

A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.

Observation. The following was observed in the outdoor play space: -Prickly weeds are growing into the playground through the chain link fence. -The back gate is not in working condition and unable to be closed and secured. This is a concern as there is a parking lot adjacent to the playground.

Corrective action. Ensure a healthy a safe environment is provided at all times.

A licensee must: (b) have an emergency plan that sets out procedures to prepare for, mitigate, respond to and recover from any emergency.

Observation. The facility has prepared emergency kits on site. Both the food and water in these kits has expired.

Corrective action. Ensure the emergency supplies are sufficient to implement the emergency plan.

A licensee must keep, for each child, a record showing the following information: (a) name, sex, date of birth, medical insurance plan number and immunization status.

Observation. Licensing reviewed four children's files. One did not include the child's immunization status.

Corrective action. Ensure all children's files are complete.

Observation. Criminal Record Check not obtained.

Corrective action. Ensure all staff files are complete.

A licensee must keep, for each child, a record showing the following information: (a) name, sex, date of birth, medical insurance plan number and immunization status.

Observation. One or more children did not have a medical insurance plan number on file.

Corrective action. Ensure all children's files are complete.

A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.

Observation. The east and south sides of the outdoor playground have large amounts of thorny weeds growing through the chain link fence and into the playground. In some areas these thorny weeds stretch three feet into the play space posing a risk to the children in care.

Corrective action. Ensure a safe environment is provided at all times.

A licensee must keep current records for each child showing (a) the information set out in subsection (2), (b) if applicable, the information and agreements set out in subsections (2.1) and (2.2), and (c) the consents referred to in subsection (3).

Observation. Licensing reviewed three children's files. One of these files was missing the child's start date and the name and number of the child's medical practitioner. One of these files was missing the child's MSP number.

Corrective action. Ensure all children's files are complete.

Observation. Licensing reviewed two staff files. One file did not contain a valid criminal record check. The facility is in the process of applying for an updated criminal record check.

Corrective action. Ensure all staff have a valid criminal record check at all times.
