A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.

Observation. The following were observed during inspection: -Green table in the play room is visibly dirty with dark spots on it -Walls are dirty and paint coming off. It appears that children are peeling off the walls - There are areas covered with clear tape. The tape is dirty and posing risk off germs contamination - Broken bikes in the outdoor play area ( repeated contravention) - Chairs and tables in the outdoor area have grime and dirt on it - There are holes and missing patches of grass in the playground.

Corrective action. Please ensure that facility is clean and in good repair while children are in attendance.

A licensee must ensure that children do not have access to any object or substance that may be hazardous to the health or safety of a child.

Observation. The following items were accessible to children: -Plastic bags and personal grooming items in the washroom - Ants out product beside the toilet

Corrective action. Please ensure to provide healthy and safe environment to children while in care.

A licensee must: (b) have an emergency plan that sets out procedures to prepare for, mitigate, respond to and recover from any emergency.

Observation. Expired food and water in emergency bin. Additionally, insufficient supplies to respond to emergency situation.

Corrective action. Licensee will update the supplies.

A licensee must keep, for each child, a record showing the following information: (i) a photograph or digital image of the child, and other information that can be used to readily identify the child in an emergency.

Observation. Review of 4 children files observed one child was missing photo on file.

Corrective action. Licensee will update children's file.

A licensee must keep, for each child, a record showing the following information: (a) name, sex, date of birth, medical insurance plan number and immunization status.

Observation. Review of 4 children files observed one child was missing immunization status on file.

Corrective action. Licensee will update the files.

A licensee must keep, for each child, a record showing the following information: (c) daily attendance record, indicating for each day whether the child is absent or, if the child is present, the time of arrival and departure.

Observation. Incomplete attendance records. None of the children were signed out for last 2 days.

Corrective action. Please ensure that arrival and departure time is recorded accurately on the attendance sheet.

A licensee must ensure that any heated water accessible to children is heated to no more than 49° Celsius.

Observation. Hot water accessible to children in the bathroom measured over 49° Celsius.

Corrective action. The licensee will check the thermometer she has on site and correct the temperature.

A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.

Observation. It was observed in the outdoor space that there were some rusty, dirty, and broken toys, bikes, and play structures.

Corrective action. The licensee will clean the toys and discard broken ones.

A licensee must establish a program to instruct children in, and to practise the rules of, health and hygiene. Observation (CORRECTED DURING INSPECTION): There is a diaper mat that was covered with plastic that was torn and taped in several parts which makes it ineffective to clean and sanitize.

Corrective action. The Licensee removed the plastic cover during the inspection. The diaper changing mat inside the cover is in good shape.

The licensee must not permit a person over the age of 12 to be ordinarily present on the premises of a community care facility while children are present, unless the person is of good character and the licensee has obtained a criminal record check for that person.

Observation. A criminal record check for a person over the age of 12 who is ordinarily present is not available.

Corrective action. The licensee has already applied to renew, just waiting for the results.

A licensee must keep, for each child, a record showing the following information: (d) name and telephone number of a parent, medical practitioner and emergency contact.

Observation. Review of 3 children files observed one child was missing medical practitioner's name and phone number on file.

Corrective action. Ensure all children files are complete.

A licensee must have in writing from a parent, and maintain at the community care facility, consent (a) to call a medical practitioner or ambulance in case of accident or illness if the parent cannot immediately be reached.

Observation. Review of 3 children files observed one child was missing parent's consent to call ambulance in case of health emergency.

Corrective action. Ensure all children files are complete.

Observation. Review of 3 children files observed one child was missing medical insurance plan number on file.

Corrective action. Ensure all children files are complete.

Observation. Licensing observed the following during today's inspection; - A hallway closet near washroom need to be secured. -A rusted child size scooter in the outdoor play area

Corrective action. Ensure that a healthy and safe environment is provided to children in care.

A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.

Observation. -In the outdoor play space the exit gate and adjacent fence panel on the east side are rotten and falling over. Also in the outdoor play space the fence panels that run along the west side of the yard are beginning to rot and become insecure. -Along the walkway to the outdoor play space there are rotten wooden boxes tossed aside with nails exposed. -In the nap room the blind cords are hanging low and accessible to the children in care.

Corrective action. Ensure a healthy and safe environment is provided at all times.

A licensee must keep, for each child, a record showing the following information: (d) name and telephone number of a parent, medical practitioner and emergency contact.

Observation. Licensing reviewed four children's files. One file did not include the name and contact number for an emergency contact.

Corrective action. Ensure all children's files are complete.

A licensee must keep current records for each child showing (a) the information set out in subsection (2), (b) if applicable, the information and agreements set out in subsections (2.1) and (2.2), and (c) the consents referred to in subsection (3).

Observation. There was a child in care without a registration form. The Licensee had minimal information on this child on her phone.

Corrective action. Ensure to keep current records for each child showing all items identified in section 57 of the child care licensing regulations.

A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.

Observation. Licensing conducted a walk through of the indoor and outdoor spaces and observed the following: -Just outside the front door, where children enter and exit the facility at arrival and departure, and to enter and exit for outdoor play, there was a table with gardening supplies on it. This pile of tools also contained a large knife. The knife was accessible. -There is a cord running across the hallway upstairs between the nap room and the staircase. The cord is a potential tripping hazard. The Licensee and Licensing Officer discussed possible options to ensure the cord is secure. -The outdoor fence on the west side of the backyard is beginning to rot. Numerous boards are breaking away from the main parallel boards creating gaps and unsteady fencing. -In the main play room there is paint chipping exposing drywall; primarily around the two pillars in this space.

Corrective action. Ensure to provide a healthy and safe environment both indoors and outdoors at all times.

A licensee must have all of the following: (b) an emergency plan that sets out procedures to prepare for, mitigate, respond to and recover from any emergency.

Observation. The fire extinguisher on site expired in February 2021.

Corrective action. Ensure that the facility is able to respond to an emergency by having the fire extinguisher serviced annually.

A licensee must keep, for each child, a record showing the following information: (b) date of enrolment in the community care facility.

Observation. Licensing reviewed three children's files. One of these files did not identify the child's start date.

Corrective action. Ensure all children's files are complete.

A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.

Observation. The fence on the east side of the outdoor play space has begun to rot. There are boards unattached, boards falling out, the paint on almost every board is peeling and there are nails exposed. There is a second fence behind this fence that appears to be in good condition. A conversation took place with the Licensee regarding either removing the rotting fence and ensuring the second fence is adequate or replacing this fence if there is a need for two fences.

Corrective action. Ensure that facility is in good repair at all times.

A licensee must ensure that any heated water accessible to children is heated to no more than 49° Celsius.

Observation. The water in the children's washroom was 54.1 degrees celsius posing a risk to children in care.

Corrective action. Ensure that the heated water accessible to children is heated to no more than 49 degrees celsius.
