A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.

Observation. The following was observed: - no gate at bottom of stairs to upstairs patio (outdoors) - no toilet paper accessible to children - no paper towel accessible to children

Corrective action. A corrective action plan will be submitted.

A licensee must ensure that all indoor and outdoor play materials and equipment accessible to children are (b) safely constructed, free from hazards and in good repair.

Observation. Outdoor equipment dirty and some broken toys.

Corrective action. Clean equipment and remove broken toys.

A licensee must: (b) have an emergency plan that sets out procedures to prepare for, mitigate, respond to and recover from any emergency.

Observation. Insufficient supplies for emergency recovery.

Corrective action. Licensee will add necessary supplies.

A licensee must keep, for each child, a record showing the following information: (a) name, sex, date of birth, medical insurance plan number and immunization status.

Observation. Licensing reviewed eight child files, one file was missing medical insurance plan number.

Corrective action. Manager will obtain records.

A licensee must keep, for each child, a record showing the following information: (c) daily attendance record, indicating for each day whether the child is absent or, if the child is present, the time of arrival and departure.

Observation. Attendance has not been recorded since April 10, 202. This is a repeat contravention.

Corrective action. A corrective action plan will be submitted.

A licensee must keep, for each child, a record showing the following information: (d) name and telephone number of a parent, medical practitioner and emergency contact.

Observation. Licensing reviewed eight child files, three did not have doctor information and one did not have emergency contact information.

Corrective action. Manager will obtain records.

A licensee must keep current records of each of the following: (b) for each employee, the records required under section 19 (1) [character and skill requirements].

Observation. Licensing reviewed two staff files, a staff did not have a current ECE license.

Corrective action. License is being processed.

A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (a) a criminal record check for the person.

Observation. Licensing reviewed two staff files, a staff did not have appropriate criminal record check.

Corrective action. Licensee will obtain appropriate criminal record check.

A licensee must ensure that children do not have access to any object or substance that may be hazardous to the health or safety of a child. Observation (CORRECTED DURING INSPECTION): There was air freshner under the washroom sink and the cupboard was not locked.

Corrective action. The Manager removed this at the time of inspection.

Observation. There are some fence panels missing from the backyard fence. The Manager stated they were hoping to get funding but have not applied for it yet. This is a repeat contravention.

Corrective action. A corrective action plan will be submitted.

A licensee must keep current records of each of the following: (c) a record respecting compliance with section 22 (2) (b) and (c) [emergency training and equipment].

Observation. There was no evidence of monthly fire drills or the annual emergency drill. The Manager said she has done them but not recorded them.

Corrective action. A corrective action plan will be submitted.

A licensee must keep, for each child, a record showing the following information: (a) name, sex, date of birth, medical insurance plan number and immunization status.

Observation. Some files were missing care card numbers

Corrective action. The Manager will correct this

A licensee must keep, for each child, a record showing the following information: (c) daily attendance record, indicating for each day whether the child is absent or, if the child is present, the time of arrival and departure.

Observation. Attendance has not been recorded since April 17/24. This is a repeat contravention.

Corrective action. A corrective action plan will be submitted.

A licensee must keep, for each child, a record showing the following information: (k) the date on which the child stops attending the community care facility.

Observation. End dates have not been recorded for children who have left.

Corrective action. The Manager stated they will correct this.

A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (a) a criminal record check for the person.

Observation. One staffs criminal record check is not site specific

Corrective action. The Manager will apply for one.

A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (c) a record of the person's work history.

Observation. A record of the person's work history was not obtained for one staff.

Corrective action. The Manager will correct this

A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (b) character references in respect of the person.

Observation. One staff was missing references

Corrective action. The Manager will correct this

A licensee must ensure that all indoor and outdoor play materials and equipment accessible to children are (b) safely constructed, free from hazards and in good repair.

Observation. The following was observed outside: -the yard is enclosed by a wooden fence -there are some fence panels showing signs of wear and tear -there are gaps between some of the wood slats within some panels -there are loose slats within some panels -some parts of the slats and panels appear rough and require sanding or new wood

Corrective action. Please ensure that the enclosure for the yard is in good repair and prevents injury to children in care

A licensee must keep, for each child, a record showing the following information: (c) daily attendance record, indicating for each day whether the child is absent or, if the child is present, the time of arrival and departure.

Observation. Attendance records are only available until April 21, 2023.

Corrective action. Please ensure that attendance is taken daily with arrival and departure times for each child.

A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (b) character references in respect of the person.

Observation. One staff's file did not have references completed.

Corrective action. Please ensure all staff have their references checked prior to hiring them.

A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (f) evidence that the person has complied with the Province's immunization and tuberculosis control programs.

Observation. Immunization status was not obtained for one employee.

Corrective action. Please ensure all staff have immunization status on file.

A licensee must ensure that each employee (a) is trained in the implementation of the fire drill system and emergency plan described in subsection (1), including the use of any equipment noted in the fire drill system and emergency plan.

Observation. The fire extinguisher required servicing before February 8, 2023.

Corrective action. Please ensure that the fire extinguisher is serviced once a year.

A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.

Observation. The following was observed outside in the yard: #1) -it appears that the licensee has tried to secure the fence that encloses the backyard with makeshift pieces of wood -there are some gaps where there are pieces of wood that have been attached to cover them -along the bottom part of the fence in some areas, there are pieces of wood that have been attached to prevent the fence from leaning -the licensee explained that it was coming loose and parts of the fence were leaning -a secure fence line is required to ensure safety of children in care #2) -it appears one of the gutters on the roof is not working properly and the water is falling in the path where parents and children access the facility -there is so much water accumulated in the ground in that area that it has created a hole which is full of water -the licensee has attempted to fill the hole but the pressure of the water falling continues to create a hole -this pool of water is not safe as it presents a potential falling hazard, potential drowning hazard and potential standing water hazard which could facilitate mosquito breeding -this area needs to be addressed immediately

Corrective action. Please ensure the outdoor play area is a safe and healthy environment. Please provide a plan for both the fence and hole of water to ensure the safety of children in care.

A licensee must display in a prominent place in the community care facility both the licence, including any terms or conditions of the licence, and the name of the manager.

Observation. Manager's name is not posted in the facility.

Corrective action. Please ensure that the Manager's name is posted in the facility.

A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (a) a criminal record check for the person.

Observation. The Licensee's criminal record check has expired.

Corrective action. Please ensure all criminal record checks are up to date.

A licensee must ensure that children have at all times immediate access to an employee who (a) holds a valid first aid and CPR certificate, provided on completion of a course that meets the requirements of Schedule C.

Observation. The staff that provides transportation to some children in the facility does not have a valid first aid certificate.

Corrective action. Please ensure that children have immediate access with someone who has valid first aid.

A licensee must ensure that each piece of furniture or equipment for sleeping (c) is appropriate for the size, age and development of the children intended to use it.

Observation. An infant in care was napping in a bouncy/rocking chair with a blanket covering the entire thing. The infant is new to the facility and had trouble transitioning to a crib. The Licensee stated they tried to use the crib but the infant was upset and could not fall asleep. The parent stated that the infant may sleep better in the chair.

Corrective action. Please ensure that all children use sleeping furniture that is appropriate and safe for the age of the child. A bouncy/rocking chair is not suitable for naptime.
