  Observation. Licensing observed an open S-hook on the right hand side of one infant swing in the Group Child Care (Under 36 Months) outdoor play space. The chain that is attached to the S-hook was coming through the gap. Manager stated that this swing would not be used until the S-hook was fixed.

  Corrective action. Ensure that all outdoor equipment accessible to children is safely constructed, free from hazards and in good repair.

  Observation. Licensing observed five staff files this date. One file did not have a certificate that was validated through the ECE Registry.
