  Observation. Licensing audited four staff files. Two files did not contain character references.

  Observation. Licensing performed an audit of three staff files this date. Three files were missing an immunization record.

  Corrective action. Ensure staff files are complete; including an immunization status.

  Observation. Licensing performed an audit of five children's files this date. On file was missing a doctor and contact information.

  Corrective action. Ensure children's files are complete; including a doctor and contact information.

  Observation. Licensing observed that the facility tested positive for lead in the water. No corrective action plan has been submitted to Licensing. Manager stated that a two minute flush of the taps was being performed daily.
