    Observation. Licensing audited five children files. One file did not contain an immunization status.

    Corrective action. Ensure the immunization status of children are obtained and kept in record.

    Observation. Licensing observed the attendance record showed arrival times. The departure times were missing several times in October and November 2025.

    Corrective action. Ensure both the arrival and departure times of the child are kept in the daily attendance record.
