Observation. One child was in a playpen resting in a separate nap room without supervision. The Licensee stated that a video baby monitor has been ordered for the sleep space and that employees physically check on the child during nap time. Children must be supervised at all times to reduce health and safety risks. Ensure all children are supervised at all times. Provide a response to Licensing with evidence of compliance by February 2, 2026. One of the multi-age programs was operating with more than 3 children under 36 months. The Licensee indicated that the sleep space in the one multi-age room is more quiet, so the younger children from both multi-age programs have been put down for rest time in this space. The Licensee stated that this only occurs while children are resting and that children are divided appropriately during the rest of the day. Exceeding group maximums outlined in Schedule E puts children at risk. Children are to be divided into groups with each group having no more children than permitted in Schedule E. Provide a response to Licensing with evidence of compliance by February 2, 2026.

A licensee must keep, for each child, the name and telephone number for a medical practitioner as part of maintaining current records. Observation. Records indicate the last fire drill was practiced in March 2025. Fire drills must be practiced monthly to ensure staff and children are prepared to respond in the case of an emergency. The Manager indicated that a drill had occurred in April, but was not recorded on the log sheet. Ensure the fire drill system in practiced and recorded monthly. Submit a response to Licensing with evidence of compliance by May 30, 2025.

Observation. One employee record was not available for Licensing to view at the time of the inspection. Employee records must be on site as outlined in Legislation. Ensure current records for all employees are kept on site. Submit a response to Licensing with evidence of compliance by May 30, 2025.

Observation. There is no current log of daily play space inspections. Daily, monthly and annual plays space inspections are required to reduce risk to children by ensuring the environment is safe and appropriate. Ensure the facility maintains a log of daily, monthly and annual inspections which includes corrective actions taken by facility employees. *This remains an outstanding contravention from January 30, 2025. Submit a response to Licensing with evidence of compliance by May 30, 2025.

Observation. The table used for eating in one of the multi-age programs has damaged/chipped paint on the eating surface. This poses a potential risk for ingestion if flecks of paint contaminate children's food. Additionally, the eating surface is not smooth and cannot be properly cleaned/sanitized. Ensure facility furniture is safe, easy to clean and free from hazards. The cleanliness and sanitation of the facility and equipment is below minimum standards. During the inspection, Licensing Officers observed: - Visible dirt and debris in the diapering area of one of the multi-age rooms. - Food, dirt/debris on eating surfaces such as counters and high chairs in one of the multi-age rooms. - Moldy food was seen on the counter and in the refrigerator in one of the multi-age rooms. - Water bottles used for drinking in one of the multi-age rooms are unclean. - The sink in one of the multi-age rooms was filled with dirty dishes. Empty dirty dishes were also seen stored in the refrigerator and on the counter. Ensuring the facility is clean and sanitary is required to provide a healthy and safe environment for children. Ensure the facility, furniture and equipment is clean. Hazardous objects and substances were identified as accessible to children: - A dirty, sharp knife was on the kitchen counter in one of the multi-age rooms and was accessible to children. - Bottles of "Febreeze" air and linen refresher were observed in one of the multi-age rooms and was accessible to children. These items pose a risk to children for injury or illness. Ensure children do not have access to any object or substance that may be hazardous to their health or safety. *This remains an outstanding contravention from January 30, 2025. A new climbing structure in the outdoor play space is taller than 3 feet in height. Equipment taller than 3 feet may pose a risk of injury to toddlers. Ensure equipment for toddlers does not exceed 3 feet in height. There is no protective surfacing below a climbing structure in the outdoor play space. This poses a potential risk for injury to children. Ensure there is protective surfacing below climbing structures to reduce the risk for injury Submit a response to Licensing with evidence of compliance by May 30, 2025.

Observation. - 2 sharp knives were located in a low unlocked drawer in the kitchen which was accessible to children. This poses a risk to children. Ensure knives are stored out of reach and inaccessible to children. - Electrical cords which are plugged into outlets are visible and within reach of children. A phone charger was plugged into an outlet, with the cable and head of the cable laying on the floor in a play area. Electrical cords pose a potential risk for shock/burns and entanglement. Ensure electrical cords are not accessible to children.

Observation. - First Aid certification is expired for one employee who works alone with children for a period of time each day. Children must have access to an employee who holds valid first aid and CPR certification to ensure the employee is prepared to respond in the case of an emergency, and reduce risk for children. Licensing Officers were informed that the employee is scheduled to re-certify within one week from the date of this inspection. Ensure children have access to an employee with valid first aid and CPR at all times during operating hours. - Employee records could not be located by the Licensee at the time of the inspection. The Licensing Officer was told that the records are kept on site. Records are required to ensure that employees working with children have the appropriate certifications/training/qualifications and experience needed to provide safe quality care, which reduces risk to children. Ensure records are kept on site and available for review upon request.

Observation. One Early Childhood Educator (ECE) certificate expired in March 2022. This is an outstanding contravention from the previous routine inspection. ECE certification is required for Multi-age programs as per schedule E. Another Early Childhood Educator certificate is expiring May 22, 2024. Ensure that persons are qualified to operate the program.

Observation. One multi-age program does not have a qualified educator as the ECE certification has expired (March 2022). This was identified as a contravention at the last inspection. Staff qualifications are required as per Schedule E to ensure employees have the skills necessary to meet children's needs and not put children at risk. Ensure children are supervised by qualified employees.

Observation. The cleanliness and sanitation of the facility and equipment is below minimum standards. During the inspection, Licensing Officers observed: - Visible dirt and debris in the diapering areas. - Sticky buildup/dirt/debris on floors, toys and surfaces. - Food, dirt/debris on eating surfaces such as counters and high chairs. - A strong animal odour Establish a cleaning system facility furniture, equipment and fixtures are clean. Fire extinguishers tags note the last date of inspection was November 2022. Equipment that is not regularly inspected may not be in working order which poses a risk for safety in an emergency. Ensure equipment is regularly inspected and in good repair. A pane of glass leaning against a wall was protruding into the walkway of a high traffic area, posing a risk for injury. The glass was moved during the inspection. Tall, heavy shelving units that are filled with equipment, toys and supplies are unsteady and not secured to the wall. This poses risk to children who may try to climb or retrieve items from the shelves. Ensure furniture accessible to children is sturdy and secure. A knob is missing off a low kitchen cupboard accessible to children. The screw from the knob is exposed and has a jagged edge. This poses a risk for injury to children. Ensure facility fixtures are in good repair. Several electrical outlets were accessible to children. This poses a safety risk. Ensure outlets are not accessible. A hallway which has been identified as an emergency exit is blocked by gates and items on both ends (i.e. four vehicle tires, boxes and a sleep cot where a child naps). This poses a potential risk in the event of a fire or other emergency. Ensure emergency exits are clear. Cots and bedding in one of the program areas appeared soiled and were being stored together so that linens and blankets were touching. To ensure a healthy environment, establish a system to ensure sleeping supplies are clean and kept separate for each child. Hazardous objects and substances were identified as accessible to children: -Two axes. - A spring action rat trap is deployed under the bathroom sink in one of the program spaces and is accessible to children. - Several products labelled "Keep out of reach of children" including, i.e. cleaning products, dog and fish food, creams and lotions, toiletries, air fresheners and scented wax melts, potting soil, sunscreen and essential oils were observed throughout both programs. - Two open bottles of alcoholic beverages were observed on the bottom shelf of the refrigerator. These items were moved during the inspection. - A cupboard containing cleaning products was unlocked. The cupboard was locked during inspection. - An unlocked drawer in the kitchen of one of the program spaces contains sharp knives and scissors. Establish a system to ensure that children do not have access to hazardous objects or substances. - Broken toys and furniture were observed in the outdoor play space. One item was removed at the time of the inspection. - One high chair tray is cracked and broken. Establish a system to ensure unsafe equipment is repaired, removed or replaced to reduce the risk of injury to children. The Licensee advised that they do not perform or record monthly or annual facility inspections. Establish a system for monitoring play spaces, equipment and furniture in order to ensure there is no risk to children.

Observation. Upon review of facility records, it was determined that a reportable incident occurred at the facility which was not reported as per Schedule H of the Child Care Licensing Regulation. Submit a report to Licensing for the incident which occurred on December 11, 2023. Licensing Officers reviewed all child records. Some child records were incomplete missing information such as: Immunization status, physician contact information and emergency contacts. This poses a potential risk to children in the event of an emergency. Establish a system to ensure child records are complete and accurate.

Observation. The fire drill records were reviewed from the past several years as they are kept together on a bulletin board in the facility. There appear to be several months of records missing from 2023 and 2024. The Licensee indicated the missing records are at the facility somewhere, but they were not able to be located at the time of the inspection. Ensure current records are are available indicating that each employee practices implementing the fire drill system at least once each month.

Observation. Staff records of character and skill requirements were not available at the time of the inspection. Ensure current records are kept on site for each employee and contain the required information as outlined under section 19 of the Child Care Licensing Regulation.

Observation. During the inspection, the Licensing Officers observed children being left unsupervised while they were eating snack. This poses a risk to children in the event of an emergency such as choking or allergic reaction. Ensure children are supervised at all times.

Observation. There were 4 children under 36 months being provided with care in one of the Multi-Age groups. A Licensee left four children in the indoor play space unsupervised, to collect their dog from the outdoor play space.

Observation. One of the Licensee's does not hold a valid Criminal Record Check. The Licensee who open's the facility on their own, does not hold a valid first aid and CPR certificate.

Observation. Some registration forms were incomplete. A child's registration form was a photocopy of their sibling's registration form, with only the name and date of birth changed. Another child's registration form was missing immunization information. It is noted that the child with the photocopied registration form no longer attends the facility. A current photo or digital image of each child was not available to be reviewed by the Licensing Officer. The monthly play space inspections have not been recorded since July 2021.

Observation. Lead in drinking water results have not been provided to the Licensing Authority. The diaper mat is in poor repair, with exposed foam making it difficult to adequately clean.
