    Observation. A review of staff records indicates that verification of educator certificates by the Director of the Early Childhood Educator Registry is not documented in any staff file. Verification of educator certificates is required to ensure that educators possess the qualifications, skills, and training necessary to work with children. Submit evidence by June 18, 2026, confirming that all educator certificates have been verified.

  • C4.1 - Are facility records current and complete?Non-critical

    Observation. A review of children's records identifies the following deficiencies: All children's records lack a section for parents to identify individuals who are not permitted access to their child. Three children's records do not contain the name and telephone number of a medical practitioner. One child's record does not include immunization status. One record does not contain written parental consent authorizing the facility to contact a medical professional. All children's records do not include additional physical characteristics that could be used to readily identify a child in an emergency. Children's records must contain complete and current information to support the health, safety, and identification of children in the event of an emergency. Submit evidence by June 18, 2026, demonstrating that all children's records have been updated and contain the required information. A child with a medical diagnosis requiring the use of an inhaler does not have a documented care plan on file. A care plan is required for children with medical diagnoses to ensure educators are aware of the child's needs and understand when intervention may be necessary. Submit evidence by June 18, 2026, confirming that a care plan has been developed in consultation with the child's parent. Review of play space inspection records indicates that daily inspections are not documented on several days in June. Daily play space inspections are required to ensure that the environment remains safe and free of hazards for children. Submit a written response by June 18, 2026, outlining the measures that will be implemented to ensure daily play space inspections are consistently completed and recorded.

  • C7.1 - Is the facility safe, sanitary, free from hazards, in good repair, and appropriate for the age and development of children?Non-critical

    Observation. A slide is positioned in an area that does not maintain the required six-foot clearance zone and is located in close proximity to wooden structures and fencing. Maintaining a six-foot clearance around play equipment is considered best practice to reduce the risk of injury and ensure children do not fall onto or collide with adjacent equipment or structures. Submit evidence by June 18, 2026, demonstrating that the slide has been relocated or repositioned to provide a minimum six-foot clearance on all sides. Although educators conduct daily play space inspections, a thorough inspection checklist has not been developed. A thorough inspection checklist is necessary to ensure all staff understand and consistently assess all required components of the play space. Submit by June 18, 2026, the thorough daily, monthly, and annual play space inspection checklists which have been created.

    Observation. One employee file was found to be missing proof of the employee's qualification as a responsible adult. Evidence of at least 20 hours of relevant education is required.

    Observation. An inspection of employee files revealed that three of the four employee files were missing valid criminal record check results. All employees must have current criminal record check results at all times. Results must be obtained and provided to Licensing immediately.

    Observation. Results of a recent test for radon have not yet been received. Results must be forwarded to Licensing upon receipt.

    Observation. One gate has a gap greater than 3.5" under it. A plan to lower the gate exists, and will be implemented now that the snow is gone from that part of the yard.
