    Observation. LO observed bleach bottles and other cleaning supplies stored in a cupboard underneath the bathroom sink. The cupboard door had a child safety lock installed, however during this visit the safety lock was not in use.

    Observation. LO observed that a diaper change mat in the washroom is beginning to show signs of wear and tear, with a small hole in the cover. It also appears either soiled or stained around the seams.
