  Observation. LO observed that children's files are not complete. This issue continues to be in non-compliance.

  Observation. LO observed that staff files are not complete. LO will provide Child Care Staff List form via email. This issue continues to be in non-compliance.

  Observation. Review of children's files shows that some have conflicting information about where immunization status would be, with some children missing medical insurance as well as doctors information.

  Observation. While fire drills are posted, they do not appear to be practiced monthly - some months do not have any information showing drills completed.

  Observation. LO observed that the facility does not have an Active Play and Screen Use Policy.
