  Play materials and/or equipment are not safely constructed, free from hazards and in good repair. [CCLR Sec. 16(4)(b)]

  Observation. LO observed the following in the outdoor play area: • play equipment such as a slide and playhouse are covered in mud and in need of a cleaning • turf/carpeting has become loose and poses a tripping hazard

  There is an emergency plan that sets out procedures to prepare for, mitigate, respond to and recover from any emergency. [CCLR Sec. 22(1)(b)]

  Observation. LO observed that the centre's emergency supplies do not contain enough food and water to support staff and children for up to 3 days. LO to provide the VCH child care emergency preparedness guide "Prepare to Care".

  A record for each child containing the name, sex, date of birth, medical insurance plan number and immunization status is not kept. [CCLR Sec. 57(2)(a)]

  Observation. LO observed the following in the children's records: • 4 children are missing immunization status • 1 child is missing a PHN/Care Card number

  Fire drills are not practiced at least once each month. [CCLR Sec. 22(2)(b)]

  Observation. LO did not observe evidence that fire drills are practiced monthly. The centre's emergency drill record sheet is missing fire drills for October 2025 - current.

  All indoor and outdoor play materials and equipment accessible to children are suitable for the age and development of the children, safely constructed, free from hazards and in good repair. [CCLR Sec. 16(4)]

  Observation. There is a gate at the hallway that leads to the stairs, there is a large gap at the bottom which children could access, there is direct access to the hallway from the Gross Motor Room. Outside, the gate, and the fence gate are both broken and do not properly latch.

  The licensee keeps a current record for each child showing the information required. [CCLR Sec. 57]

  Observation. Children's files are available, but are missing emergency contact that is not parent and immunization status.

  Employees employed as early childhood educators or assistants hold the appropriate certificates issued under Division 2, which must be verified by the licensee using a system established and maintained by the director of the early childhood educator registry [Employee Qualifications].  [CCLR Sec. 19(4)(a)]

  Observation. Staff file is complete with the exception of proof of validation of staff ECE certificate.
