Observation. Originally found out of compliance on January 14, 2026. Licensing observed the door remains unlocked. The Licensee currently uses a classroom located on the second level of the building without a monitoring system to provide staff information of who may be entering the building. Licensing is concerned there is a risk to children's safety.

Observation. Originally found out of compliance on January 14, 2026. Licensing observed that the Licensee has made alterations to the the types of mattresses that are being used in the cribs. There are the following: - cribs with a mattress as well as a mat on top of the crib mattress used for sleeping- the crib mattress and mat are different sizes. - a thick mat which is often used when children sleep on the floor is inside of a crib instead of the original mattress. The program uses bedding that is oversized for the size of the mattresses, which is loose on top and the sides of the mattress. Licensing is concerned there is an increased health and safety risk to the children due to these issues.

Observation. Licensing observed the following: The facility was not following a specific cleaning and sanitizing procedure during the inspection. The cribs that are used for the children are located next to decorated bulletin boards, which have items stapled to the board. Licensing is concerned there is a safety risk as children are at the level they have access to the bulletin board and the items stapled to the board. There is a air conditioner located in the classroom that is no longer necessary during the colder months of the year. Licensing has a concern that there is a risk of injury due to the large item being stored in the main play area and not in use.

Observation. Licensing observed that some of the mattresses are covered in cloth rather than plastic, Licensing is concerned the cloth covered mattresses are creating an opportunity of cross contamination when wet and the inability to clean the mats effectively if the they become soiled.

Observation. LO observed that the gymnasium was set up with children's high chairs, chairs, tables and a shelf containing children's lunch bags. Staff stated since December they have been using the gymnasium for meal times. LO informed staff that the gymnasium is not a licenced space therefore should not be used by children and staff.

Observation. In the monthly attendance record, LO observed that the facility had a power outage to which staff stated that the facility had to close for the day. Licensing was not notified of the facility closure and an incident report was not provided.
