  Observation. An adult who resides at the premise does not have evidence of a criminal record check. All persons over the age of 12 who reside at the premise must obtain criminal record clearance. Submit to licensing by November 21, 2025, evidence the missing criminal record check has been obtained.

  Observation. Two child files are missing immunization status. Attendance record arrival times were incomplete at time of inspection. This was resolved during inspection. Daily play space inspections are incomplete. This was resolved during inspection.

  Observation. Heated water exceeds acceptable limit.

  Observation. Licensing did not receive notification of the facilities structural change plans prior to construction. A notification of floor plan change has been received and the new floor plan was verified during this inspection.
