Observation. It was alleged that children were not being supervised at the time that a reportable incident occurred at the facility. Based on the incident report that was submitted to licensing, and conversations with the manager and a staff who confirmed that children were not supervised at the time that a reportable incident occurred, the allegation has been substantiated. Submit to licensing by August 28, 2025, information detailing how you will ensure children are supervised at all times.

Observation. A review of the current written polices and procedures that are intended to guide employees in the care and supervision of children was conducted. A supervision policy was not included in the polices and procedures staff are given upon commencing employment at the facility. The licensee must provide staff with polices and procedures that are intended to guide staff in the care and supervision of children. Through conversation with the manager and a review of facility records, it was confirmed that a parent was not immediately notified that their child was involved in a reportable incident. Observation. It was observed the vinyl plank flooring located in the kitchen and eating area has wide gaps and is lifting, causing a tripping hazard. It is the responsibility of the licensee to ensure the facility is in good repair for the safety of the children. Submit to licensing by August 28, 2025, evidence the vinyl plank flooring has been repaired and is no longer a tripping hazard.

Observation. It was observed that two active care plans have not been reviewed yearly. Establish a system to ensure care plans are reviewed yearly. A medication form for a child who requires daily medication is incomplete. Establish a system to ensure medication forms are complete and contain detailed information regarding the medication.

Observation. One educator certificate was not verified by the licensee using a the system established by the director of early childhood educator registry. Ensure all staff certificates are validated.
