  Observation. One ECE certificate is not verified by the licensee using a system established and maintained by the early childhood educator registry. Incomplete staff files increase risk to children. The contravention was cleared during the inspection.

• C7.1 - Is the facility safe, sanitary, free from hazards, in good repair, and appropriate for the age and development of children?Non-critical

  Observation. A tall doll house located in the playroom is unstable. The doll house poses a risk to children if they were to attempt to climb or push the doll house over. Submit a response to licensing by January 13, 2025, with evidence of how the identified risk has been mitigated.

• C6.1 - Are acceptable hygiene practices maintained?Non-critical

  Observation. No covered container is available in the children's bathroom to contain soiled diapers or pull ups. A covered container for soiled diapers/pull ups is a requirement to ensure the health and hygiene of children. Submit a response to licensing by January 13, 2025, with evidence of having obtained a covered container to use for soiled items.

• C4.1 - Are facility records current and complete?Non-critical

  Observation. Two child files are missing the name and phone number of a medical practitioner. Incomplete child files increase risk to children in an emergency. Submit a response to licensing by January 13, 2026, with evidence of having obtained the missing information. Two of the six children present at time of inspection have not been signed into the facility on the attendance record. Incomplete attendance records may put children at risk in an emergency. Submit a response to licensing by January 13, 2026, that demonstrates how you will ensure children's attendance records are maintained to reflect the accurate number of children present at all times. A care plan is not available for a child who has a severe anaphylactic allergy. Incomplete child records increase risk to a child who requires extra support in the facility. Submit a response to licensing by January 13, 2026, that includes evidence of having developed a care plan for this child.

• C10.2 - Are children supervised, and are employee to children ratios maintained?Critical

  Observation. It was alleged that children were not being supervised at the time that a reportable incident occurred at the facility. Based on the incident report that was submitted to licensing, and conversations with the manager and a staff who confirmed that children were not supervised at the time that a reportable incident occurred, the allegation has been substantiated. Submit to licensing by August 28, 2025, information detailing how you will ensure children are supervised at all times.

• C2.1 - Are written policies and procedures in place to guide employees in all matters regarding the care and supervision of children?Non-critical

  Observation. A review of the current written polices and procedures that are intended to guide employees in the care and supervision of children was conducted. A supervision policy was not included in the polices and procedures staff are given upon commencing employment at the facility. The licensee must provide staff with polices and procedures that are intended to guide staff in the care and supervision of children. Submit to licensing by August 28, 2025 evidence that a supervision policy has been developed and staff have been provided a copy of the policy.

• C4.1 - Are facility records current and complete?Non-critical

  Observation. Through conversation with the manager and a review of facility records, it was confirmed that a parent was not immediately notified that their child was involved in a reportable incident. It is the responsibility of the licensee to ensure parents are immediately notified when a child is involved in a reportable incident. Submit to licensing by August 28, 2025, a response that includes information on how you will ensure parents are immediately notified if their child is involved in a reportable incident.

• C7.1 - Is the facility safe, sanitary, free from hazards, in good repair, and appropriate for the age and development of children?Non-critical

  Observation. It was observed the vinyl plank flooring located in the kitchen and eating area has wide gaps and is lifting, causing a tripping hazard. It is the responsibility of the licensee to ensure the facility is in good repair for the safety of the children. Submit to licensing by August 28, 2025, evidence the vinyl plank flooring has been repaired and is no longer a tripping hazard.

  Observation. The managers name was not displayed in a prominent place in the facility. Ensure the managers name is posted in a prominent location in the facility.

• C4.1 - Are facility records current and complete?Non-critical

  Observation. It was observed that two active care plans have not been reviewed yearly. Establish a system to ensure care plans are reviewed yearly. A medication form for a child who requires daily medication is incomplete. Establish a system to ensure medication forms are complete and contain detailed information regarding the medication.

• C3.1 - Does the facility have sufficient employees who possess adequate training and experience to meet the care, supervision, and needs of the children?Non-critical

  Observation. One educator certificate was not verified by the licensee using a the system established by the director of early childhood educator registry. Ensure all staff certificates are validated.

• C1.2 - Does the licensee operate the facility in a manner that will promote the health, safety and dignity of persons in care?Non-critical

  Observation. Radon results have not been submitted to licensing. Submit the Radon test results to licensing when you receive them back from the lab.
