    Observation. - The last entry in the log of minor incidents is August 30, 2022. One of two employees were able to locate these forms upon request. The manager determined that there was a minor accident in September 2022. - Some employees are not aware of, or trained to implement a child's care plan. The plan does not contain sufficient information to guide employees in the care of the child and the plan is not readily accessible to staff. - The same care plan was developed in September 2021 and is overdue for an annual review. - Facility records are being kept in three different offices which required additional effort to access the necessary information during this inspection.
