Observation. There is no covered container for soiled pull ups and diapers in the Group Child Care (30 months to school age) bathroom. A covered garbage container prevents the spread of germs and diseases and control odors. Observation. A flood occurred in the lower level of the facility while the facility was closed for summer vacation from June 30, 2025 to July 13, 2025. Licensing was informed of the flood on Monday July 14, 2025, an incident report was requested and was submitted on July 14, 2025. A restoration company has been contracted and has begun remediation work. The Group Child Care (Under 36 month) program was relocated over the weekend of July 12th and 13th into the Group Child Care (School Age) classroom. The Group Child Care (School Age) program was relocated to an unlicensed offsite location location on Monday July 14, 2025 without licensing approval. The licensee was informed that a new facility license is required in order to operate the Group Child Care (school Age) program at another location. As of July 15, 2025 the Group Child Care (School Age) program is not in operation. Submit to licensing by July 22, 2025, a floor plan change notification for the Group Child Care (Under 36 Month) program to operate out of the Group Child Care (School Age) classroom. Include an updated floor plan with the notification.

Observation. At time of inspection the Group Child Care (Under 36 months) classroom is not in operation and a restoration company has installed drying equipment. The Group Child Care (under 36 months) program has relocated to the Group Child Care (School Age) classroom which is located on the same level of the affected Group Child Care (Under 36 Months) classroom. The licensee has not provided licensing with evidence this classroom was unaffected and is safe for children to be in. Submit to licensing by July 23, 2025, evidence the Group Child Care (School Age) classroom is unaffected by the flood and is safe for children.

Observation. A review of the children's files was conducted, some of the files were found to be incomplete, items missing included doctor's name, phone number, and immunization status. Incomplete child files may increase risk to children. Submit to licensing by July 22, 2025, a plan that outlines how you will ensure children's files are complete.

Observation. A review of the facilities completed medication forms was conducted, one medication form did not indicate what type of medication was administer to the child. Incomplete medication administration forms may put the child who requires the medication at risk. Submit to licensing by July 22, 2025, a plan that outlines how you will ensure medication administration forms are complete prior to staff administering the medication.

Food that may not be stored at food safe temperatures could put children at risk of illness. Observation. The complaint information alleged that two children from the Group Child Care (Under 36 Months) program were combined with the Group Child Care (30 Months to School Age) program for a period of time on May 1, 2025. Based on statements from staff, complainant and a review of child files, it was confirmed that two children ages 22 months and 24 months old were combined in the Group Child Care (30 Months to School Age) program for a period of time on May 1, 2025. Operating below the minimum standards of group sizes and employee to child ratios puts children at risk. By May 21, 2025 inform licensing how you will ensure that children will be divided into groups at all times, with each group having no more children than permitted in Schedule E of the Child Care Licensing Regulation. The complaint information alleged that employee to child ratios were not maintained in the Group Child Care (30 Months to School Age) program for a period of time on May 1, 2025 Based on the information gathered, and confirmed by the manager, a practicum student who is not an employee of the facility was working in ratio and supervising children for a period of time on May 1, 2025. Operating below the minimum standards of group sizes and employee to child ratios puts children at risk. By May 21, 2025, inform licensing how you will ensure the child to staff ratios will be maintained at all times.

Observation. It was observed that licensing was not notified of changes made to the Group Child Care (30 Months to School Age) outdoor play space. It is a requirement that licensing be informed prior to making any changes to the outdoor play space to ensure the outdoor play space is safe and free of any potential hazards. By May 28, 2025 inform licensing how you will ensure that licensing is notified of any changes made in the information provided under Section 9 (applying for a license) of the Child Care Licensing Regulation. Additionally submit an outdoor play space notification to licensing that contains details of the changes that have been made and the plans of any additional changes.

Observation. While conducting the complaint inspection the licensing officer identified the following additional contravention: Licensing was not notified of a change of manager. Submit a written plan to licensing that demonstrates how licensing will be notified of any changes made to Schedule B of the Child Care Licensing Regulation.

Observation. It was alleged that a parent was not immediately notified of injuries their child sustained while at the facility. Through conversations with the manager and a review of the facility's minor incident log book it was confirmed that parents are not immediately informed of injuries that have occurred at the facility. This was substantiated. Submit a written plan to licensing that demonstrates how parents will be immediately informed of illness or injures at the facility. While conducting the complaint inspection the licensing officer identified the following additional contravention: The facility has not been keeping a current record for each child showing any notification of a parent, emergency contact or medical health officer made under Child Care Licensing Regulation Section 55 -notification of illness or injury. Submit a written plan to licensing that demonstrates how the facility will ensure there is evidence of having notified parents of illness or injury. It was alleged the facility did not have measures in place for children who act aggressively towards other children at the facility. Through conversations with the manager and a review of facility records it was confirmed that care plans have not been completed for children identified as requiring extra support for behavior management. This was substantiated. Submit a written plan to licensing that demonstrates how the facility will maintain compliance with Child Care Licensing Regulation Section 58-care plans, and the system that will be implemented when children are identified as requiring extra support at the facility.

Observation. While conducting the complaint inspection, the Licensing Officer identified the following additional contravention: There was no evidence of a fire drill being practiced in June 2024.

Observation. It was alleged that a staff used her hands over a child's hands restricting the child's body autonomy as a means of obtaining compliance with a task. This was done while the child was crying and physically resisting. It was further alleged this child was held in place by the staff's legs so the child could not move away from the staff. This was substantiated. It was alleged a staff physically restrained a child's forearms as a means of gaining the child's compliance. This was substantiated. While conducting the complaint investigation, the Licensing Officer identified the following additional contravention: A staff removed a child in a manner that violated the child's body autonomy and disregarded the child's right to control their own body. While conducting the complaint investigation, the Licensing Officer identified the following additional contravention: A staff pushed on a child's lip as behavior guidance strategy following an incident where the child had bitten the staff. A Health and Safety plan was requested by the Licensing Officer to address the allegations of harmful actions. The Health and Safety plan submitted by the Licensee on June 24th, 2024 addressed how risk will be mitigated during the investigation. The Health and Safety plan was accepted by Licensing on June 24, 2024 and will remain in effect until the Licensee is notified in writing that it will no longer be required.

Observation. It was alleged the facility does not provide children with a comprehensive and coordinated program of activities that meet the developmental needs of the children. During the inspection the Licensing Officer observed an inadequate amount of play material and supplies that meet the developmental needs of the children. Additionally it was confirmed by staff there was no system of program planning being implemented at the facility. This is substantiated.

Observation. While conducting the complaint inspection, the Licensing Officer identified the following additional contraventions: An area carpet located in the Group Child Care (30 months to school age) classroom is curled causing a tripping hazard. Two wood pallets located in the outdoor play space are splintered and have nails sticking out of them. Sharp metal wire was exposed on a large metal ring located in the facilities front entrance. It was alleged that a staff was using an electric weed trimmer in the outdoor play space while children were present. This posed a potential risk of flying objects that may harm children. This was substantiated. It was alleged that children play on playground equipment while at public community parks that exceed the Director of Licensing Standard of Practice-Safe Play Space requirement of a maximum five feet in height. This was unsubstantiated.

The complaint alleges that an educator's certificate is not posted in the facility. - At the time of this inspection, Licensing observed that this educator's certificate was not displayed in the facility. Observation. At the time of the inspection it was observed that an Early Childhood Educator Assistant (ECEA) was working alone with children in the 30 Months to School Age program during the first two hours of operation. The Child Care Licensing Regulation requires that a 30 Months to School Age program be supervised by an Early Childhood Educator (ECE) at all times. By reviewing the staff schedules for February and March, as well as through interviews with both the ECEA and the facility manager, it was confirmed that the ECEA was regularly scheduled to work alone in lieu of an ECE. As a result of this investigation, contravention of Section 34 (2) (b) Group sizes and employee ratios has been Substantiated.

Please ensure that staff are recording all play space inspections.

Observation. Discussed the allegation that children from the Infant Toddler program were cared for in the 30 mos-School Age program on March 2, 3, and 4. Manager confirms this to be true. Complaint Substantiated. Discussed the allegation that on February 28 a Responsible Adult (RA) acting as an Early Childhood Educator Assistant took three children down to the lower floor of the facility during naptime, while the manager remained with the sleeping children on the upper floor. Manager was able to identify who the RA was that worked with her on this day, but did not recall that this staff person went downstairs with the children. Complaint Unable to Substantiate.
