Observation. There is a carpet in the eating area that appears visibly dirty however, the licensee reports that she washes it weekly. This carpet has curling edges creating a tripping hazard. When the LO entered the facility there was a strong smell of cigarette smoke present. The Licensee had the windows open and there was a can of air freshener present. The licensee reported that the upstairs occupants smoke and it comes through the vents.

This door is the only entrance and exit to this room through the daycare. The carpet in the eating / play / nap area was very dirty. The back shed was not locked. Phone charger cord hanging out of wall, accessible to children. There are several windows in the space. The blinds are drawn, windows shut and plastic covering the outside of the window preventing natural light from entering and any fresh air.

Observation. There were several pieces of apple on the floor around the play room. There was some old Kleenex and other debris on the floor. There is a restrictor on the door near the bathroom instead of the loose bungee but the door can still be partially opened creating a pinching hazard. The towel used for cleaning in the bathroom was on a hook within children's reach. There is no evidence of a thorough cleaning and sanitizing program for this facility. The door to the upstairs can be opened several inches creating potential for injury.
