    Observation. The licensee had purchased bi-fold door restrictors, however, they broke. The bi-fold doors continue to present a pinching hazard. There is a garbage can in the main play room that does not have a lid or cover. The hallway to the washroom and nap room from the designated play space has areas that are not to be used by the children. There are doorways and areas that are accessible but not suitable to children.

    Observation. There was visible debris ( food) on the high chair in the playroom. There are several bi-fold doors that do not have restrictors. The licensee demonstrated how she places a block under the door to prevent the doors from opening, however, this was ineffective. Outside of the playroom, accessible while walking down the hall to the bathroom and nap room are doors that are open to areas where children are not permitted.

    Observation. The licensee walks school age children across the street to the school in the middle of the block through the lane. In the back yard the orange fencing does not completely cover all of the hazards and is falling down in places. There are gas cans and tools accessible to children. The inside playroom has no windows to allow for fresh air to enter the play space. There is children's food / snack left out on the table. There was visible debris on the table and highchair.

    Observation. A partially filled child's milk / baby bottle was observed on the floor in the play room. Food was out on the table, no child was sitting eating it, there were also some food bits on the vacant high chair. A child's chair at the table has a cracked seat. Some toys were broken, sharp edges, accessible to children. The backyard has broken fence panels. The lawn mower is accessible to children. There is a broken window, accessible to children, near the cement stair case. There is a bottle of insecticide just outside the door that children use to access the playground. The door to the front door entrance area and upper level of the house was open.
