Observation. An application to amend the license to include a change of a manger was sent to Licensing originally Nov. 2022, however, details on the form were not correct. A corrected form was requested, next form still contained errors. A correct form was requested but not yet received.

Observation. The ground surfacing in the playground, under and near climbing equipment is packed down or has shifted so that the loose fill material is no longer effective. This has been noted on prior inspections.
