    Observation. The heater in the south room was very hot to the touch. In one nap room there are loose cords hanging done and accessible to children. The landscaping fabric is exposed causing a tripping hazard in the wood chips area of the yard.

    Observation. There are two chairs that have torn and worn fabric coverings making it impossible to adequately clean and sanitize.

    Observation. There is a black chair with a vinyl type covering, it is worn. This piece of furniture would not be able to be properly cleaned or sanitized. There are bi-fold doors in a nap / play room that do not have restrictors and could be a pinching hazard.

    Observation. There is insufficient sand in the sandbox, the landscaping fabric is visible. The area around the sand box originally was wood chips but now there are very few wood chips, the landscaping fabric is visible and creates a tripping hazard, there is a large amount of sand on the ground and on the ground in the playhouse structure. This area is uncovered and creates an area where animals could pollute the materials with their waste.
