  Observation. The current posted license does not accurately reflect the current manager. This has been previously cited.

  Ground Surfacing: The depth of loose fill material (protective surfacing) meets or exceeds the minimum requirements of the Standard of Practice. [Director of Licensing Standard of Practice - Safe Play Space]

  Observation. The ground surfacing in the playground, under and near climbing equipment is packed down or has shifted so that the loose fill material is no longer effective. This has been noted on prior inspections.

  Observation. The door to the furnace / janitorial room adjacent to the bathroom was open and the door would not shut properly or lock. There was no thermometer in the fridge.
