    Observation. There is insufficient sand in the sandbox, the landscaping fabric is visible. The area around the sand box originally was wood chips but now there are very few wood chips, the landscaping fabric is visible and creates a tripping hazard, there is a large amount of sand on the ground and on the ground in the playhouse structure. This area is uncovered and creates an area where animals could pollute the materials with their waste.

    Observation. The heating vents are very hot to the touch and are brown which indicates elevated temperatures. In the kitchen in the lower area is a gate blocking children's access to the kitchen, however, there is a drawer immediately adjacent to the gate, accessible to children that had sharp items in it. Gates do not close adequately. It was not evident how the potties are cleaned and sanitized appropriately.
