Corrective action. The staff will obtain a first aid certification that meets the requirements of Schedule C. In the meantime, the manager will ensure children will have access to an individual with a valid first aid certification.

Observation (CORRECTED DURING INSPECTION): In one of the under 36 months program, plastic bags were accessible to children and pose a strangulation hazard. Upon notifying a program staff, the plastic bags were removed and are no longer accessible to children. Corrected during inspection.

Observation. 1 child missing doctor name and number. Licensing and the Manager discussed walk-in clinic as sufficient information.
Corrective action. The Manager will follow up with the child's family to obtain this information.

- EQUIPMENT AND FURNISHINGS
A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees. Observation (CORRECTED DURING INSPECTION): In 2 nap rooms (under 36 months programs), the following was observed: -in 1 nap room, there is a table next to a crib. On this table are toys and equipment that have not been stored in a secured manner. There is a potential for these items to fall onto a child in the adjacent crib. -in the other nap room, there is also a table with a pile of toys and equipment, including a large folded tent that is leaning against the wall and table. There is a potential for these items to fall onto a child. On the 2nd day of the inspection, Licensing observed that the toys and equipment in both nap rooms have been appropriately addressed by securely storing items and/or storing equipment elsewhere. Corrected during inspection.
Corrective action. Licensing and the Manager discussed alternate ways to store these items in both nap rooms. The Manager plans to either store these items elsewhere and/or in a secure manner, such as underneath the tables instead of on top.
11010 - CCLR s.13(1) - EQUIPMENT AND FURNISHINGS
A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.
Observation. In one of the under 36 months and 30 months to School Age programs, there are multiple dents exposing the dry wall and areas of chipping paint. For the 30 months to School Age program, the Manager explained because the cots are stored against this wall, sometimes the cots hit the wall causing these dents and chips. Licensing and the Manager discussed storing the cots elsewhere or implementing a protective surface on this area of the wall. This is a repeat contravention.
Corrective action. Please submit a plan outlining how these areas will be addressed.
11020 - CCLR s.13(2) - MEDICATION
If a licensee has agreed with a parent to give a child any medication prescribed by a medical practitioner or provided by the parent, the licensee and his or her employees must ensure that the medication is (b) readily accessible to employees.
Observation. Upon reviewing the facility’s allergy list and child’s file, there is written instructions from the parents to administer medication in case an allergic reaction occurs. As per the Manager, there is no medication administration form as the parents have not provided the medication. Licensing and the Manager discussed following up with the parents regarding this allergy and the medication information provided on the child’s file.
Corrective action. The Manager will follow up with the family to obtain the medication and medication administration form.
16020 - CCLR s.53(1)(b) - RECORDS AND REPORTING
A licensee must keep, for each child, a record showing the following information: (h) any special instruction respecting the child's diet, medication, participation in a program of activities, or other matter relevant to the child's care, (i) given by the child's parent to the licensee in writing.
Observation. In discussion with the Manager and after review of children’s files, there are 4 children with documented allergies. For all 4 children, the follow-up instructions are unclear if the child ingests the allergen. As per the Manager, the families communicated that these were mild reactions and no medication is required. By the 2nd day of inspection, the Manager connected with 2/4 families.
Corrective action. The Manager will follow up with the families to clarify the allergies and follow-up instructions in case an allergic reaction occurs.

A licensee must ensure that children do not have access to any object or substance that may be hazardous to the health or safety of a child. Observation (CORRECTED DURING INSPECTION): In one of the under 36 months programs, Licensing oberved that a staff's hoodie was hanging over a child's crib. There is a potential that personal items, such as medication, may be in the staff's hoodie pocket that would be accessible to a child. Upon notifying the staff, the hoodie was immediately placed on a hook that is out of children's reach.
Corrective action. Please ensure that that children do not have access to staff belongings.
11200 - CCLR s.17 - EQUIPMENT AND FURNISHINGS
A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.
Observation. Across the 6 programs, Licensing observed multiple walls with dents and chipping paint which are porous and uncleanable. This is a repeat contravention. In one of the under 36 months programs, Licensing observed 2 child-sized chairs (yellow and red) with chair legs displaying chipping paint and rust.
Corrective action. Please ensure furniture, equipment, and fixtures within the facility are in clean and good repair.

Observation. Upon reviewing 4 staff files, Licensing observed the following: -1 staff file with an expired validation and 3 character references without documentation of the references being verified. As per the Manager, she is uncertain if the previous Manager had contacted this staff's references upon hire. -Another staff file without a resume and an ECE validation that does not match up with the current license on file.
Corrective action. Please ensure all staff records are complete.

A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.
Observation. *Repeat violation of FIR #SMOY-BZKU8U (29/Mar/2021)* 1) Some interior walls had chips & spots w/ worn paint, which are porous & uncleanable. 2) The rubber ramp cover (outside) was peeling on 1 side, which is a tripping hazard.
Corrective action. The manager stated that: 1) These areas can be repaired within a few weeks. 2) She'll speak to the building manager re. repairs.
11020 - CCLR s.13(2) - POLICIES AND PROCEDURES
A licensee must (a) ensure that a community care facility has (i) emergency exits that meet the requirements of the BC Building Code, and (ii) a fire drill system that is approved as definedby section 31 of the Fire Services Act.
Observation. Fire extinguishers were last serviced in 2020/Aug.
Corrective action. Fire extinguishers require annual servicing, by the date indicated on the service tag.
13010 - CCLR s.22(1)(a) - RECORDS AND REPORTING
A licensee must immediately notify a parent or emergency contact if, while under the care or supervision of the licensee, the child (a) becomes ill or is injured.(1.1) In the case of a licensee who provides a care program described as Child-minding, the parent to be notified under subsection (1) is the parent who is on the premises on which the child-minding service operates.
Observation. Based upon discussions w/ the manager, parents aren't always notified immediately when children become injured, unless it appears more serious, &/or blood is present.
Corrective action. Parents or emergency contacts must be notified immediately when children becomes ill or injured, so they have the opportunity to pick-up their child ASAP, if they wish.
19010 - CCLR s.55(1)(a)(1.1) - RECORDS AND REPORTING
The licensee must (a) develop the care plan in consultation with a parent of the child requiring extra support and any person requested by the parent.
Observation. Care plans for children requiring extra support were unavailable.
Corrective action. Care plans must be created in consultation w/ parents, & posted in the children's program rooms.
19250 - CCLR s.58(2)(a) - STAFFING
A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (a) a criminal record check for the person.
Observation. *Repeat violation of SMOY-AXAR9U (28/Mar/2018), SMOY-BL529G (23/Jan/2020), & SMOY-BZKU8U (29/Mar/2021)* CCFL reviewed all employees' files. 3 employees lacked CRCs & 1 employee's CRC was addressed to another organization.
Corrective action. All requirements of s. 19(1) must be obtained by employees' 1st day of employment, which includes CRCs & CRC shares. The manager showed CCFL evidence of her recent CRC applications for these 4 employees. In the meantime, these employees can't work alone w/ children or assist w/ toileting/diapering.
12040 - CCLR s.19(1)(a) - STAFFING
A licensee must keep current records of each of the following: (b) for each employee, the records required under section 19 (1) [character and skill requirements].
Observation. CCFL reviewed all employees' files. 1 employee lacked references. The manager explained that 2-3 references were checked for this employee, but the employee took the documentation afterwards.
Corrective action. All requirements of s. 19(1) must be kept onsite @ all times.
12430 - CCLR s.56(1)(b) - STAFFING
The licensee must not permit a person over the age of 12 to be ordinarily present on the premises of a community care facility while children are present, unless the person is of good character and the licensee has obtained a criminal record check for that person.
Observation. An ordinarily present individual lacked a CRC.
Corrective action. The manager showed CCFL that she recently applied for this individual's CRC.
12160 - CCLR s.20(1)
March 29, 2021Routine Inspection3 items noted
- EQUIPMENT AND FURNISHINGS
A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.
Observation. 1) The facility has been in operation for a few years and some of the interior walls are beginning to show signs of wear and tear (chips in the walls and areas where paint is worn off exposing the raw building material). These areas are now porous and are no longer cleanable. 2) The ramp leading from the building to the outdoor play area is covered with a black rubber material. This material is worn away in areas and is beginning to lift causing a tripping hazard.
Corrective action. 1) Licensing requests a written plan of action by April 19, 2021 outlining when the work for the walls will be completed. The time line for completing this issue is June 30, 2021. 2) Licensing requests the repair to rubber surfacing be completed by April 19, 2021.
11020 - CCLR s.13(2) - EQUIPMENT AND FURNISHINGS
A licensee must ensure that children do not have access to any object or substance that may be hazardous to the health or safety of a child. Observation (CORRECTED DURING INSPECTION): During the inspection of the various program rooms licensing observed medication stored on a shelf above the activity sink and in drawers by the activity sink - all of which are accessible to the children in care. Licensing also noted in one program room two personal belongings of staff within reach of the children. The Manager closed the cupboard with the child safety latch and relocated another item out of children's reach. NOTE: the contravention of having personal items accessible to children was previously identified on facility inspection report #SMOY-B9AS75 and #SMOY-BL529G. The Manager indicated she reviewed this issue with the staff last week.
Corrective action. The Manager removed all medications from the program rooms and stored them out of children's reach within their office and personal belongings were secured within the program room during the inspection.

Observation. The Manager informed licensing she has identified staff files that require updating with individuals criminal record clearance letters addressed to the Licensee. The Manager shared with the writer the various correspondence she has had with the criminal records review agency regarding this issue. The Manager is fully aware of needing to have current records on site and is actively working to address this issue. Licensing acknowledges the Manager is undergoing some administrative challenges with their account with the criminal records review agency however this doesn't preclude the facility from having current staff records on site. NOTE: the need to have current records on site for staff was previously contravened on facility inspection report #SMOY-BL529G.
Corrective action. Ensure all staff have current records on site including criminal record clearance letters.

- EQUIPMENT AND FURNISHINGS
A licensee must ensure that children do not have access to any object or substance that may be hazardous to the health or safety of a child. Observation (CORRECTED DURING INSPECTION): In the preK 3 room the Licensing Officer observed a purse and bag of personal belongings accessible to children. These items need to be inaccessible to all children as there is the potential they contain hazardous items such as over the counter medication. This contravention was previously coded in Feb 2019 - facility inspection report #SMOY-B9AS75.
Corrective action. The Licensing Officer spoke with the Manager during the inspection and these items were relocated to a cupboard and a child safety lock was used on the cupboard doors.
11200 - CCLR s.17 - EQUIPMENT AND FURNISHINGS
A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.
Observation. In the preK 1 room the Licensing Officer noted the entry door is in need of repair as there are areas on the door where the paint is peeling off. In addition to this observation the Licensing Officer also noted a small brown chair, located in the nap room, is showing signs of wear and is no longer cleanable as areas have worn away and are now porous.
Corrective action. The Manager will need to repair or replace the above noted items.
11020 - CCLR s.13(2) - EQUIPMENT AND FURNISHINGS
A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.
Observation. The Licensing Officer observed a climbing structure outdoors that was partially located on resilient ground cover. One of slides ended on a portion of the rubber matting while the other slide ended on artificial turf.
Corrective action. The Director of Licensing Standards of Practice - Safe Play Spaces references height of climbing structures and the applicable type and depth of resilient surfacing these structures must have to reduce the risk to children. The Licensing Officer provided a copy of this document to the Manager for their reference. The Manager will need to remove the structure or make it inaccessible until such time they are able to meet the requirements in the Safe Play Spaces document.
11010 - CCLR s.13(1) - HYGIENE AND COMMUNICABLE DISEASE
A licensee must establish a program to instruct children in, and to practise the rules of, health and hygiene. Observation (CORRECTED DURING INSPECTION): The Licensing Officer noted there were no paper towels either of the paper towel dispensers in the children's washroom between classroom prek 3 and prek 2.
Corrective action. During the inspection this issue was brought to the Manager's attention and when the Licensing Officer and Manager returned to the children's washrooms the paper towel dispensers were refilled.
15030 - CCLR s.46(1) - PROGRAM
A licensee must provide to children a comprehensive and coordinated program of indoor and outdoor activities that (b) is appropriate for the age and development of children in each group in the community care facility.
Observation. Through discussion with staff, and the Manager the Licensing Officer determined that there is insufficient outdoor play material to meet the needs of all children in care. Both the group child care (under 36 months) and the group child care (30 months to school age) programs share the outdoor space.
Corrective action. Ensure there is sufficient outdoor material to meet the number of children enrolled as well as all of the children's developmental needs. For example: additional ride on toys, outdoor sand table, hockey sticks and balls etc.
18020 - CCLR s.44(1)(b) - STAFFING
A licensee must keep current records of each of the following: (b) for each employee, the records required under section 19 (1) [character and skill requirements].
Observation. Licensing reviewed 3 staff files and noted that one file did not contain a current criminal record clearance letter. The file did have a criminal record clearance letter addressed to another agency. The Manager stated they had applied for the results to be shared with them and are waiting for the results.
Corrective action. Ensure all required staff documentation is current prior to staff working with children.
12430 - CCLR s.56(1)(b) - STAFFING
A licensee must not employ a person in a community care facility unless the licensee is satisfied, based on the information available to the licensee under subsection (1) and the licensee's or, in the case of an employee who is not the manager, the manager's own observations on meeting the person, that the person (a) is of good character.
Observation. The Licensing Officer reviewed 3 staff files and noted that two of the files did not contain any evidence that the Manager/Licensee verified the staff's references. The Manager stated she uses a standardized form when speaking with the references however she had not done so for the files pulled.
Corrective action. Ensure all staff's references have been verified and implement a system to indicate this has been conducted.
