The licensee must (b) review the care plan at least once each year with a parent of the child requiring extra support and any person requested by the parent.
Observation. In the 30 months to school age program, a care plan was not annually reviewed.
Corrective action. The manager will follow up with the parents to review the information on the care plan. If information remains the same, then a new date of review and parents signature will be obtained. If there are any changes, then a new care plan will be completed.

A licensee must ensure that children do not have access to any object or substance that may be hazardous to the health or safety of a child. Observation (CORRECTED DURING INSPECTION): Licensing observed the following items accessible in the 3-5 children's washroom: -2 bottles of Lysol spray -1 box of disinfecting wipes -1 bottle of air freshner Upon notifying the Manager, these items were placed elsewhere that is not accessible to children.
Corrective action. Please ensure hazardous objects or substances are not accessible to children.

A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (a) a criminal record check for the person.
Observation. Upon reviewing staff files, Licensing observed an expired criminal record check for the Manager. As the Manager is also the Licensee, the renewal of the CRC will need to be done through Fraser Health. Licensing will email the access code along with this report.
Corrective action. Please ensure all staff files are complete with a valid criminal record check.

- RECORDS AND REPORTING
A licensee must keep, for each child requiring extra support, a current care plan showing the following information: (a) the diagnoses relevant to the child's requirement for extra support, as made by health care professionals.
Observation. As per staff, there are two children in the IT program that require an inhaler. There is an administration of medication form available for both children, but neither have a care plan. As per staff, the inhaler is administered when they see that the child physically needs it and/or when parents tell them to administer it at drop-off. A care plan is needed for both children that reflects what symptoms to look out for, what medication to administer, the amount of medication to administer, and a photo of the child.
Corrective action. Please ensure that a care plan is developped for both children in consultation with the family and/or health care professionals.

A licensee must keep, for each child, a record showing the following information: (b) date of enrolment in the community care facility.
Observation. Upon reviewing 8 childrens files, all 8 were missing an enrolment date.
Corrective action. Please ensure that children's files are complete with all necessary information.

A licensee must keep current records of each of the following: (b) for each employee, the records required under section 19 (1) [character and skill requirements].
Observation. Upon reviewing 6 staff files, Licensing observed the following: -2 staff files are missing ECE registry validations and 2 staff files contain expired validations -1 staff is missing 2 references and the Manager is missing 3 references -Manager's file is missing a resume
Corrective action. Please ensure that all staff files are complete with required documentation.

A licensee must ensure that each employee (b) practices implementing the fire drill system at least once each month.
Observation. In the 3-5 program, Licensing observed that a fire drill had not been recorded for the month of Nov.2022. As per the staff, a fire drill was not conducted during this month as the program got busy.
Corrective action. Please ensure that a fire drill is practiced and recorded monthly.

- EQUIPMENT AND FURNISHINGS
A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.
Observation. Outstanding from the previous facility inspection report #SMOY-BP3Q5B the facility's floors were needing to be repaired or replaced as they are wearing and is porous. This item remains outstanding.
Corrective action. The Manager made arrangements to have the work completed on the floors however due to COVID-19 and the challenges with the supply chain the work remains outstanding. The Manager will continue to secure the services to have the work done.
11010 - CCLR s.13(1) - RECORDS AND REPORTING
A licensee must keep current records of each of the following: (f) a log of minor accidents, illnesses and unexpected events involving children, that did not require medical attention and were not reportable incidents described in Schedule H.
Observation. Upon review of children's files there was one file that had a child's immunization status and no registration or emergency consent card on file. The child's sibling has been attending and the family thought they only needed to complete one form for both children.
Corrective action. The Manager reminds all families the facility requires all applicable information prior to the child being left in care. The Manager will communicate this information with the family today.

A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (a) a criminal record check for the person.
Observation. Upon review of staff files it was noted one file had a criminal record clearance on file that was addressed to another agency/employer.
Corrective action. The Manager will apply to have the results shared. In future the Manager will ensure all applicable documentation is on site prior to the employee providing care to children.

- EQUIPMENT AND FURNISHINGS
A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.
Observation. 1) there are areas within the facility (hallway on main floor at the top of the stairs and in the activity areas on the lower floor) where the flooring is showing signs of wear. These areas have the top layer of the floor worn away and is now porous. 2) the outside area has a slide and climbing structure that does not have resilient ground cover. The Director of Licensing Safe Play Spaces document states: for equipment that is over 45 cm but is less than 1.5 meters, protective surfacing is recommended. Protective surfacing such as soft sand, wood chips, pea gravel or rubber material will reduce the risk of injury from falls.
Corrective action. In discussion with the Manager there are plans in place to address the protective surfacing and to replace the flooring. At the present time the outdoor areas is not being used by the children in care.
