• C1.1 - Does the licensee continually inform licensing of any significant changes to the structure or operation of the facility?Critical

  Observation. Licensing was informed of the manager change that occurred last year, however the name of the new manager was not posted. The manager's name must be displayed in a prominent location.

• C2.1 - Are written policies and procedures in place to guide employees in all matters regarding the care and supervision of children?Non-critical

  Observation. It was reported that fire drills are practiced monthly with the school, however the drills have not been recorded in the fire drill log since April of 2024. A method of ensuring that fire drills are practiced by each employee monthly, and that those practices are recorded is required. A log of emergency plan practices indicates that the last emergency plan was practiced in January of 2023. A system for ensuring that emergency plans are practiced by each employee annually, and that those practices are recorded is required.

• C4.1 - Are facility records current and complete?Non-critical

  Observation. An inspection of the attendance records showed that the times of departure were missing on some days. A method of ensuring the times of arrival, departure and absences is required.

• C3.1 - Does the facility have sufficient employees who possess adequate training and experience to meet the care, supervision, and needs of the children?Non-critical

  Observation. Character references for one staff person were not available for review during this inspection. Evidence that character references have been obtained is required.

• C2.1 - Are written policies and procedures in place to guide employees in all matters regarding the care and supervision of children?Non-critical

  Observation. Records indicate that fire drills were practiced 11 of the past 12 months.

• C4.1 - Are facility records current and complete?Non-critical

  Observation. New registration forms for the school-age program are missing some required information. Registration forms for all programs did not include written consent to seek emergency medical care. A log of daily play space inspections exists, however a two week period in February is missing confirmation that the inspections took place.

• C3.1 - Does the facility have sufficient employees who possess adequate training and experience to meet the care, supervision, and needs of the children?Non-critical

  Observation. One staff's CRC result letter was missing from the file.

• Has the facility been tested for radon gas?Non-critical

  Observation. Manager will be contacting School District 10 to confirm whether radon testing was conducted during the winter months of 2020-2021.
