Observation. It was alleged that documentation for minor incidents lacks evidence of parent notification. A review of the Minor Accident, Illness and Unexpected Events Log identified incidents occurring between April 2025 and November 2025 that should have been reported to parents. The manager confirmed that parents were not notified of these incidents. Failure to notify parents limits their ability to respond to their child’s health and safety. Based on the information reviewed and confirmation from the manager, this allegation is substantiated. Submit by January 27, 2026, how parents will be informed of incidents involving their children. It was alleged that licensing was not informed of reportable incidents involving children. A review of the Minor Accident, Illness and Unexpected Events Log determined that children were involved in incidents that meet the definition of reportable incidents as outlined in Schedule H. These incidents were not reported to licensing as required. Failure to report incidents prevents licensing from assessing compliance, monitoring risk, and ensuring children’s safety. Based on the information reviewed this allegation is substantiated. Submit the reportable incidents from April 2025 to November 2025, to licensing by January 27, 2026. It was alleged that documentation for a child's care plan was lacking required information. A care plan for a child requiring extra support was reviewed. The plan was created in November 2025, however, records indicate the child required additional support as early as April 2025. The current care plan does not address all of the child’s needs or outline strategies to support the child. There is no evidence the care plan was developed in consultation with the parents. There is also no record demonstrating compliance with care plan requirements. An incomplete and delayed care plan may result in inconsistent or ineffective support for the child, increasing the risk to the child’s safety, development, and well-being. Based on the information reviewed this allegation is substantiated. Submit by January 27, 2026, a current care plan that addresses all identified needs of the child. Evidence that the plan was created in consultation with the parents and how compliance with the care plan requirements will be recorded and monitored.

Observation. During the inspection, the Manager disclosed that a care plan has not yet been created for a child who requires extra support. Care plans are an important resource for staff and parents as they work toward common goals for a child. A system whereby care plans are created with parents is required.

Observation. The temperatures in the two refrigerators where children's food is stored, measured above 4 degrees Celsius. A safe holding temperature for food is below 4 degrees C. A system for regular monitoring of the temperatures is required. A gap between a fence post and a shed in the outdoor play space is greater than 3.5", creating an entrapment hazard. The gap must be smaller than 3.5".

Observation. One of the two employee files that were inspected was missing required information. The information in an employee file provides important evidence of character and experience relevant to caring for children. A method for keeping complete employee records, and retrieving them upon request during an inspection, is required.
