    Observation. There is no diaper changing surface available.

    A licensee must keep, for each child, a record showing the following information: (a) name, sex, date of birth, medical insurance plan number and immunization status.

    Observation. Licensing reviewed 6 children's files and noted that 3/6 files were missing an immunization status. The Manager stated that she will update the files to reflect the information collected from families. Licensing discussed with the Manager that a notation of whether the child is immunized or not at this time is sufficient.

    Corrective action. Please ensure that all files contain information on whether the child is immunized or not.
