Observation. Upon review of all employee records it was noted that 4 required updated validation of Qualification Certificates in order to demonstrate that the employees have adequate training and experience to carry out duties assigned. Notify Licensing of addition of this information to employee records on or by June 2, 2025. It was also noted that two employee records require a statement regarding status of Immunization for COVID-19 vaccination. In order to be informed of whether employees are vaccinated against COVID-19, a statement must be included in the employee record, so that if there is a COVID-19 outbreak in the facility, it is easily determined who can or cannot work during the outbreak. Notify Licensing on or by June 2, 2025 that status of immunization for both employees has been added to the employee records.

Observation. It was observed that one of the exits was blocked with books and other resources. At the time of inspection, medications were found to be accessible in the children's cubby area.

Observation. At the time of inspection the manager's name was not displayed in a prominent location. The licensee does continually inform licensing of any significant changes to the structure or operation of the facility by submitting the appropriate applications or notifications.

Observation. At the time of inspection, one child's record required updating to ensure special instructions regarding allergies and medications are current. - The licensee keeps a current log of minor accidents, illnesses and unexpected events. - Daily attendance records are current and complete. - The Licensee keeps a log of all play space inspections, maintenance and repairs as required.

Observation. Rubbing alcohol and rubber cement that children could potentially reach were moved during the inspection to ensure they were made inaccessible. - Exits are free and clear in the event of an emergency. - The outdoor play area was free of hazards and enclosed in a manner to ensure health and safety. - Shade areas are available during outdoor play periods. - First Aid Kits and emergency supplies are readily available to employees. - Water at handwashing sinks is an appropriate temperature to prevent injury.
