Observation. The back exit door does not open when pushing on bar. The button on the wall to release the locked bar is broken. An app on the employee's cell phone must be accessed to release the lock. Consultation with a Fire Inspector confirmed the use of the app to release the locked bar does not meet the BC Building Code. Immediate attention to this matter is required. The door must be able to be opened at all times. Submit evidence of correction by January 12, 2026.

Observation. Child Care Licensing Regulation Section 55 - Notification of illness or injury. The complaint information alleged that detailed information was not provided to the parent at the time of an injury. Additionally there was no response from the facility management when requesting follow up information. Based on the documentation provided to the Licensing Officer and discussion with management, it was confirmed notification was provided at the time of the incident and management responded to the inquiry for follow up information. The legislative requirement to notify parents when a child is injured does not include providing specific detailed information regarding the incident. Unsubstantiated. During the investigation of the complaint allegation regarding notification of an injury, the Licensing Officer reviewed the incident log book, electronic system (parent app that documents incidents), as well as speaking with the facility employees and concluded there was incomplete information in the log book for minor accidents. This contravention is a result of the inspection and not part of the complaint information allegations. C4.1 D - Child Care Licensing Regulation Section 56(1)(f) - ensure log book entries include specific details including full names and description of what occurred.

Observation. Child Care Licensing Regulation Section 52(2)(c)- Harmful actions not permitted as described in Schedule H Reportable Incidents Neglect (Definition of neglect - failure of care provider to meet the needs of a child including food, shelter, care or supervision). The complaint information alleged a failure to provide adequate supervision which may have contributed to a significant injury to a child. Based on the information provided during the investigation including speaking with an employee who observed the incident, there was no clear evidence that a lack of supervision contributed to the severity of the injury. Unable to substantiate. Also, the complaint information alleged a failure to provide adequate care as a child was without appropriate clothing on numerous occasions without gloves or boots during cold and wet weather. This resulted in ice cold hands and child in wet shoes for the day. Based on the information provided during the investigation it was confirmed by facility management to have occurred. The facility management team spoke with the employees regarding expectations to ensure children are appropriately dressed. At the time of the inspection, an employee stated that the one set of the child's gloves were misplaced in another child's cubby and could not be found. Facility gloves were not provided to the child. Substantiated. C10.1Y - Child Care Licensing Regulation 52(2)(c) - Ensure children are not subjected to neglectful actions including failure to provide basic care needs.

Observation. The following is a contravention found during the inspection and not related to the complaint. While investigating the complaint regarding nutrition, the Licensing Officer observed young children being placed in high chairs and provided a snack without prior handwashing. C6.1B - Child Care Licensing Regulation 46(1) - Ensure children wash their hands prior to eating.
- C5.1 - Are healthy eating and nutritional habits promoted and practiced?Non-critical
Observation. It was alleged that parents are not informed of all the food and drink provided to the children. The Licensing Officer spoke with employees in all four of the Group Child Care Under 36 months programs. The facility uses an electronic system (App) to communicate with the parent. The App has the ability to capture both food and drink that has been provided and consumed by the children. It was confirmed that not all programs were completing the fluid intake for children who are on formula or milk. Complaint Substantiated - contravention to Section 48(6) of the Child Care Licensing Regulation. C5.1G - Ensure parents are informed of all food, including fluid intake of formula or milk. Staff admitted there was a period of time when the electronic system was not working due to tablet issues. This has since been resolved however, the Licensing Officer suggested the facility consider a back up system in case this occurs again. One of the Group Child Care Under 36 months rooms keeps a paper file of what the children drink (milk or formula) and the amount to be provided. This list is not up to date and not all children were on the list potentially leading to children not consuming his/her required formula/milk intake for the day. Knowing formula/milk intake is especially important for children under the age of two as their main source of nutrition may be coming from the fluids as they are just transitioning to solid foods.

Observation. The following is a contravention found during the inspection and not related to the complaint. The Licensing Officer reviewed the paper log book for minor accidents, illnesses and unexpected events and found it to be incomplete (not all information recorded and unclear (in one case) if parents were informed). The matter was discussed with management. C4.1D Ensure the log book is complete and parent notification occurs.
