  Observation. One employee record did not have COVID-19 immunization status on file. This was resolved during inspection and no further action is required.

  Observation. Upon discussion of indoor active play plans, it was noted that the facility's active play policy/procedure requires further information to address when the program cannot get outside due to weather/air quality etc. Licensing Officer and manager discussed some examples, and Licensing Officer will provide resources with report. Upon review of the fire drill record, it was noted that drills were not practiced two months this year, to date. Discussed that fire drills must be practiced and documented monthly.

  Observation. As noted above, the fire drill record reflected that fire drills were not performed monthly, as required.

  Observation. During inspection it was noted that a deep freeze is located in one of the spaces used for activities, and is not made inaccessible. Discussed methods of making this freezer inaccessible to children.
