Observation. Upon review of documentation it was noted that a reportable incident was not submitted to Licensing within 24 hours. Licensing Officer will provide the Incident Report information to the Licensee. Upon review of a sampling of children's records it was noted that some care plans are incomplete. Fire drill records, Emergency Preparedness practice/material review records and maintenance log are all up-to-date and appear complete.
