  • C3.1 - Does the facility have sufficient employees who possess adequate training and experience to meet the care, supervision, and needs of the children?Non-critical

    Observation. Upon review of all employee records, one is noted as missing a validation of their certificate of qualification and one is noted as missing a valid first aide. Both items have been added to the respective files. Validations of certificates are required in order to demonstrate that the Licensee has ensured that the employees possess adequate training and that their certificate is valid. An employee who may work alone is required to have a valid First Aide certificate in order to ensure children have immediate access to a person with first aide if necessary.

  • C4.1 - Are facility records current and complete?Non-critical

    Observation. Upon review of a sampling of child records one does not have a phone number for the listed medical practitioner. In order to allow the facility timely contact with the medical practitioner listed if needed, the phone number is required as part of registration information. Two programs attendance is inaccurate, as a child was not signed out of the original program, and into the program they were visiting. Attendance records must reflect when children move between programs to ensure that their attendance in the facility can be accurately tracked through-out the day and so that if emergency services has to attend, they are accurately informed of how many children are expected to be in each program. Notify Licensing of review of child records and actions taken to ensure medical practitioner information is complete and the process put in place to ensure employees sign children in and out of programs as they move through-out the facility daily, by April 7, 2026.

    Observation. During the complaint investigation employee files are reviewed to ensure all required training evidence is present and valid. Upon review of a sampling of employee records and interviews with accused employee and the Licensee, it is determined that employees do not have the required training to implement restraints identified in a care plan. Training in regards to the use of, and alternatives to the use of, restraints is required for any employee who may be required to implement a restraint as part of a care plan. Submit a plan to implement said training for employees as necessary on or by January 23, 2026.

  • C10.1 - Are children provided with a healthy and safe environment, and protected from harmful actions?Non-critical

    Observation. The complaint alleges that restraint is used as a behavioural guidance technique. At least one child has a care plan that reflects behaviour which may require a restraint to be used. A restraint may include picking up and/or carrying/moving a child from one area to another, as a form of guidance. It is determined that a child has been subjected to a restraint by an employee as a form of behavioural guidance and that the employee does not have adequate training to implement such restraints. In order to ensure the health and safety of children in care, and protect them from harmful actions, ensure that restraints identified as a guidance technique in a care plan are only implemented by employees who possess adequate training in the use of, and alternatives to the use of,restraints. Submit a plan for implementing training of employees, as necessary, to ensure that requirements are met on or by January 23, 2026.

  • C2.1 - Are written policies and procedures in place to guide employees in all matters regarding the care and supervision of children?Non-critical

    Observation. The complaint alleges that restraint is used as a behavioural guidance technique. Upon review of the current Behavioural Guidance Policy it is determined that the policy does not adequately identify what a restraint is, when a restraint is allowable through a care plan, who can apply restraints identified in a care plan or when or how restraints in a care plan are to be applied. The Behavioural Guidance policy also does not identify when an emergency restraint may be necessary, and that it is reportable to Licensing as a reportable incident. A revised Behavioural Guidance policy is required that includes information on various types of restraints and limitations, and training requirements, as outlined above. Submit the revised Behavioural Guidance Policy on or by January 14, 2026 for review by Licensing.

    Observation. Upon a review of a sampling of care plans in each room, it was noted that care plans regarding behavioural support there was no indication that parents had reviewed the plans, and there was no indication of a method of ensuring the annual review of these plans was documented. All other child registration record information appeared complete and up-to-date. Fire Drills and Emergency Preparedness Plan practices were documented for each program. Minor Incident Logs reviewed a sampling in each program, no concerns noted. Outdoor Inspection logs, reviewed a sampling, appeared complete and to be implemented regularly.
