Observation. Inspections, maintenance, and repairs of the outdoor play space are not being documented. Lack of documentation of inspections and repairs risks unnoticed hazards, which could cause unsafe conditions and potential injuries to children. Submit by March 19, 2026 confirmation that a log is being implemented for recording inspections, repairs and maintenance of the outdoor play space.

Observation. The outdoor play space gate leading to the cliff area lacks additional safety features to prevent school-age children from opening it and accessing the area. Additionally, sheds in the yard storing hazardous equipment do not have locks to restrict access. Lack of safety features on the gate and shed locks poses a risk of children accessing hazards, potentially causing falls or injuries. Safety measures are needed to ensure their protection. Submit by March 19, 2026 confirmation the concerns have been addressed.

Observation. During the inspection, Licensing reviewed a sample of the facility records for children. Records were missing the required information to readily identify the child in an emergency (height,weight,eye color,hair color,any other distinguishing characteristics). Some were missing information regarding who may not access the child. The Licensee is to revise the template used for child records and ensure all required information is included on the form. The Licensee must ensure any current registration forms are complete with all required information. Submit to Licensing by the follow up date requested a copy of the revised form to be used for future registration and confirmation all current child records are complete.

Observation. During today's inspection Licensing observed a person over the age of 12 years present at the facility. Licensing discussed the situation with the Licensee and determined a criminal record review and character reference is required prior to this person being regularly present at the facility. Submit to Licensing by the follow up date requested a plan of how you will ensure compliance.
