    Observation. The carpet in the Group Child Care (Under 36 Months) program is frayed and lifting at the edges. This may pose a potential tripping or safety hazard for children. Submit by December 2, 2025, evidence that this risk has been addressed. One floor register is pushed inward creating gaps where fingers could become pinched. This may pose a safety hazard for children. Submit by December 2, 2025, evidence that this risk has been addressed. A bifold closet door accessible to children is not secured. The points along its folding mechanism may pose a pinching risk to children. Submit by December 2, 2025, evidence that this risk has been addressed.

    Observation. It was alleged that the staff are performing wellness checks on children without families being aware of when they are performed and what they are. The manager and licensee contact confirmed that a description of what wellness checks are is not included in their illness policy and parents are not aware of when these checks are performed or what they are. Parents need to be provided with current policies and procedures which affect the care of their children. This allegation is substantiated. Submit to licensing by July 14, 2025, how parents will be made aware of what wellness checks are and when they are performed.

    Observation. The complaint received alleged that the facility employees were not implementing their illness policy which states a child can return to care with a doctor note. The Licensing Officer reviewed two doctor notes presented to the employees, the facility illness policy, and conducted interviews with two staff members and the manager. The manager confirmed the facility illness policy was not implemented. This allegation is substantiated. Submit to Licensing by May 19, 2025, a written plan of action outlining how employees are made aware of what is written in the policies and how you are going to ensure employees are implementing the policies.

    Observation. Through the investigation into the complaint, the manager and employees disclosed that a child sought medical attention after being sent home from the facility. A child seeking medical attention after leaving the facility is reportable to Licensing under Schedule H. It is the responsibility of the Licensee to understand the definitions of reportable incidents and to notify Licensing within 24 hours. This contravention is subsequent to the complaint allegations. Submit to Licensing by May 14, 2025, the completed incident report. Through the investigation into the complaint, the manager and the employees disclosed that a child was sent home due to the presence of lice in the child's hair. The employee confirmed there was no documentation in the minor accidents, illnesses and unexpected events log regarding this incident. It is the responsibility of the Licensee to ensure records are complete and employees are documenting any minor accidents, illnesses and unexpected events involving children, which did not require medical attention and were not reportable incidents described in Schedule H. This contravention is subsequent to the complaint allegations. Submit to Licensing by May 14, 2025, evidence of the incidents documented in the log book and a plan of action outlining how you are going to ensure required documentation is occurring.

    Observation. A sampling of staff records were reviewed. The Licensing Officer observed one staff did not have their certificate verified by the Licensee using a system established and maintained by the director of the early childhood educator registry. The Manager is required to submit, by the follow up date, a plan to ensure staff files are complete.

    Observation. The Manager's system to ensure the fridge temperatures are maintained at 4 degrees Celsius or below appears to be ineffective. The Licensing Officer observed a high reading of 7.5 degrees Celsius in one fridge and a non-working digital thermometer in the other fridge. This is a reoccurring contravention originally found out of compliance on March 15, 2022 and April 3, 2023, and October 26, 2023. The manager needs to submit, by the follow up date, a plan to ensure that children's food is being stored in a manner that is safe.

    Observation. A sampling of children's records were reviewed with the following deficiencies noted: The Licensing Officer observed one child's date of enrollment in the facility was missing. The Licensing Officer observed one child's record was missing other information that can be used to readily identify the child in an emergency. The Manager is required to submit, by the follow up date, a plan to ensure that children's records are complete.

  • C3.1 - Does the facility have sufficient employees who possess adequate training and experience to meet the care, supervision, and needs of the children?Non-critical

    Observation. A review of the fire drill records indicated that September's fire drill was not practiced. A discussion with the manager revealed that the emergency plan has not been practiced in the last year. The children's safety could be affected when the required emergency practices do not occur. In the event of a true emergency employees will know what the plan of action is when practices have occurred as required by the Child Care Licensing Regulations. The Manager is required to submit, by the follow up date, a plan to ensure that fire drills and the emergency plan are practiced at the required times by the employees.

    Observation. The system in place to ensure the employees practice implementing the fire drill system each month appears to be ineffective. A review of the facility records indicated three out of six months were not practiced. This is a reoccurring contravention originally found out of compliance on March 4, 2021 and April 3, 2023.

    Observation. The facilities system to ensure the fridge temperatures are maintained at 4 degrees Celsius or below appears to be ineffective. Licensing observed one fridge temperature reading 10 degrees Celsius and the other fridge reading 7 degrees Celsius. This is a reoccurring contravention originally found out of compliance on March 15, 2022 and April 3, 2023. Vinyl mats are in poor repair presenting a challenge for proper cleaning and sanitizing.     Observation. The facility does not have a system in place to ensure the fridge temperature is maintained at 4 degrees Celsius or below. The facility has several walls where the drywall is in poor repair. A wall in the nap room is unclean. Vinyl couches, chairs and mats in poor repair presenting a challenge for proper cleaning and sanitizing. A cupboard containing Borax was open and accessible to children. An indoor climber did not have the recommended six feet of clearance around it.

At the time of this inspection Licensing observed the daily attendance did not accurately reflect the number of children in attendance. Children have access to two sets of doors leading to storage areas under the building. The doors do not close properly, were not locked and could pose harm by pinching children's fingers and hands. The facility was using equipment higher than three feet in height for the toddlers in the outdoor play space and in the indoor space.
