Observation. The Licensing Officer observed the Licensee's first aid and CPR certificate were expired. Children's health and safety may be put at risk when they do not have access to a person who holds a valid first aid and CPR certificate as required. Submit by the follow up date noted the system developed to mitigate this risk to children in care.

Observation. A review of the children's records showed all records contained a photo of the child, however, all records were missing other identifying information. Facilities must keep a current photo or digital image of each child and other information that can be used to readily identify the child in an emergency. Submit to licensing, by the follow up date noted, the system developed to ensure all children's records will contain other identifying information that can be used to readily identify the child in an emergency.

Observation. The Licensing Officer observed two change pads to be in poor repair with the inner foam exposed. Exposed foam does not allow for adequate cleaning and sanitizing. Equipment used in a facility needs to be able to be cleaned and be in good repair. Submit by the follow up date noted the system developed to ensure that equipment used in the facility is in good repair.
