  Observation. Licensing reviewed eight children's files. One file did not contain permission to release the child to someone other than the parent.

  Corrective action. Ensure written permission to release the child to someone other than a parent is obtained.

• A licensee must ensure that all indoor and outdoor play materials and equipment accessible to children are suitable for the age and development of the children,

  Observation. Licensing observed that the pea gravel under the swing set was less than two inches in depth, twelve inches of depth is required to meet the standards outlined in the DOLSOP. This was corrected at the time of inspection as Licensee Contact stated that the swings would not be used until additional surfacing was added.

  Observation. Licensing reviewed two staff files. One file contained only one character reference.

  Corrective action. Ensure character references in respect of the person are obtained for all employees.

  Observation. Licensing reviewed eight children's files. Three files did not contain date of enrollment.

  Corrective action. Ensure a record is kept for each child showing date of enrollment.

• A licensee must keep, for each child, a record showing the following information: (d) name and telephone number of a parent, medical practitioner (doctor's name and number does not apply to REC only) and emergency contact

  Observation. Licensing reviewed eight children's files. One file did not contain an emergency contact and one file did not contain the name of a medical practitioner.

  Corrective action. Ensure a record is kept for each child showing the name and contact number for an emergency contact and medical practitioner.
