Observation. Two ECEA certificates were not posted at the facility. Two ECE Infant Toddler Certificates were posted of staff who do not regularly work at the facility.

Corrective action. The licensee will provide a written response to Licensing detaining their action plan to address the contravention

Observation. Children were exposed to harmful substances when staff used too high of a bleach to water ratio when mixing sanitization solution.

Corrective action. The licensee will provide a written response to Licensing detailing their action plan to address the contravention

Observation. A Criminal Record Check had one staff had expired, and Criminal Record Checks for 3 staff were not site-specific.

Observation. Character references had not been obtained for one employee, and character references had not been verified for three employees.

Observation. Two out of the 5 Early Childhood Educator staff certificates had expired.

Corrective action. The licensee will provide a written response to Licensing detailing their action plan to address the contravention.

Observation. Immunization status had not been obtained for two employees.

Corrective action. The licensee will provide a written response to Licensing detailing their action plan to address the contravention.

Observation. Three staff certificates did not have evidence of validation by the employer.

Corrective action. The licensee will provide a written response to Licensing detailing their action plan to address the contravention.

Observation. Criminal Record Checks were not available for two persons over the age of 12 who are ordinarily present at the facility.

Corrective action. Licensee will provide a written response to Licensing detailing their actions taken. A health and safety plan was requested.

Observation. More than 4 children were supervised by one ECEA staff in the Group Child Care (30 Months to School Age) program from March 30 - April 2.

Corrective action. The licensee will provide a written response to Licensing detailing their action plan to address the contravention.

Observation. The Infant/Toddler staff to child ratio was not maintained on one day when 2 ECEA staff worked with children between 7am and 9:15am.

Corrective action. The licensee discussed an action plan with Licensing in April, proposing a temporary compliance plan. Licensee will provide a written response to Licensing detailing their action plan to address the contravention.

Observation. Children were left unsupervised while eating for a short period of time.

Corrective action. The licensee will provide a written response to Licensing detailing their action plan to address the contravention.

A licensee must: (b) have an emergency plan that sets out procedures to prepare for, mitigate, respond to and recover from any emergency.

Observation. There is no written Emergency Plan available The fire extinguishers have not been serviced annually

A licensee must keep current records of each of the following: (d) written policies and procedures that are intended to guide employees in the care and supervision of children.

Observation. The licensee has not developed and/or implemented an Active Play Policy, Screen Use, Missing Child Policy, Reportable Incident Policy and use of Video cameras

A licensee must not employ a person in a community care facility as (a) an educator or an assistant unless the person holds a certificate issued under Division 2 [Employee Qualifications], which must be verified by the system maintained by the early childhood educator registry. (Educator or assistant certificate validation)

Observation. There is no evidence that staff certificate has been validated by the licensee

A licensee must replace an absent employee.

Observation. The facility manager has been absent since July 1, 2025. She is not expected to return and she has not been replaced.
