A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.
Observation. The natural wood playground structure in the group child care (30 month - SA) program shows signs of wear throughout the structure. The wooden logs are splintering and showing signs of erosion. The manager reports that the sand box area is not in use because of severe erosion.
Corrective action. Repair or replace playground equipment. A plan for maintenance or replacement to be submitted to licensing.

- EQUIPMENT AND FURNISHINGS
A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.
Observation. The following was observed during inspection: 1. Most windows located on the second floor were open approximately 3 inches for ventilation with the factory installed stopper in place at the top of the window. No secondary childproof lock or barrier was installed on any of the windows.Toy shelves and chairs are located next to openings on windows and make it possible for a child to access the factory installed stopper. 2. The drinking water dispensing machine is located at child level and has a Hot Water on demand feature that has not been disabled. A stop sign picture symbol has been placed over the button to signify to a child not to use but this is an insufficient barrier to prevent scalding.
Corrective action. 1. Ensure all windows located on the second floor of the building that are accessible to children have a child proof lock or barrier installed to prevent a fall 2. Ensure the Hot Water on Demand feature is disabled on all water dispensing machines that are accessible to children.

A licensee must ensure that children do not have access to any object or substance that may be hazardous to the health or safety of a child.
Observation. Inspection of the toddler playground found that children had access to the following: 1. A plastic shopping bag was tied loosely around an electrical outlet that did not have a locking mechanism. Children could remove the bag and the bag could become a suffocation hazard. If the bag was removed, children would have access to an electrical outlet that did not have a locking mechanism. 2. A bicycle lock is looped through the handles of the shed located in the toddler playground and could present a strangulation hazard. (This is a repeat contravention)
Corrective action. Ensure children do not have access to any object that may be hazardous to the health or safety of a child.
11200 - CCLR s.17 - POLICIES AND PROCEDURES
A licensee must: (b) have an emergency plan that sets out procedures to prepare for, mitigate, respond to and recover from any emergency.
Observation. Emergency Supplies are insufficient to implement the emergency. Some of the supplies had expired.
Corrective action. Ensure there are enough Emergency Supplies to respond to and recover from any emergency.

A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (a) a criminal record check for the person.
Observation. A review of 5 staff files found the following: -One Criminal Record Checks was not obtained. - One Criminal Record Check was expired - One Criminal Record Check on file was not site specific
Corrective action. Ensure prior to employment that all persons have a valid Criminal Record Check that is site specific. All staff must have valid criminal record check on file.

A licensee must ensure that children do not have access to any object or substance that may be hazardous to the health or safety of a child.
Observation. 1. During inspection of the Infant Toddler room, licensing observed electrical cords that were attached to an outlet and equipment on the counter were found to be hanging at child level and pose a strangulation and fall hazard. 2. A bycicle lock is attached to the chainlink fence in the toddler playground at child level and poses a strangulation hazard. The same type of bycicle lock is also left hanging from the handles of the shed in the toddler playground and also poses a strangulation hazard.
Corrective action. Ensure children do not have access to an object that may be hazardous to their health
11200 - CCLR s.17 - EQUIPMENT AND FURNISHINGS
A licensee must ensure that the entire outdoor play area is (a) enclosed in a manner that is suitable for the age and development of children, and will ensure that children are free of harm.
Observation. During the inspection of the outside facility, it was found that the Group Child Care (School Age) program had set up a play area located in the parking lot of the facility. The cones used to indicate the play are were set up alongside of the driveway into the parking lot and would not providing adequate protection from cars entering into the parking lot. The time that children were playing in this area was during a time that parents begin to pick children up from the facility and would be considered a busy time of the day.Through discussion with the facility manager it was found that the Group Child Care (30 months - school age) program also used this as a play area during the summer because it provides more shade than the licensed outdoor space. Both programs have actual licensed space on facility grounds that fits the requirements for enclosure so it was determined that this space would no longer be used for a play space for children without being properly enclosed for use by each age group and inspected and approved by licensing for use. The facility manager has notified licensing that this space will no longer be used for play by children
Corrective action. Ensure any outdoor play area is enclosed in a manner that is suitable for the age and development of children or a proposal for an away play space is submitted to licensing for approval.
11160 - CCLR s.16(3)(a) - EQUIPMENT AND FURNISHINGS
A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.
Observation. A review of the facility classroom designated for Group Care (30 months - School Age) found that one of the walls was in disrepair. The paint was chipped at the place where 2 walls joined, exposing the gyprock.
Corrective action. Ensure the facility is in good repair
11020 - CCLR s.13(2) - LICENSING
A licensee must display in a prominent place in the community care facility the certificate, or a copy of the certificate, of any educator or assistant employed to work in the community care facility.
Observation. During review of the Infant and Toddler room it was found that only some of the staff employed to work in this room had their certificates posted.
Corrective action. Ensure educators or assistants employed to work in the facility have a copy of the certificate displayed in a prominent place in the facility.
10070 - CCLR s.11(4) - MEDICATION
If a licensee has agreed with a parent to give a child any medication prescribed by a medical practitioner or provided by the parent, the licensee and his or her employees must ensure that the medication is (b) readily accessible to employees.
Observation. An inspection of medication stored in the Group Child Care (School Age) program found that 2 children with allergies had expired medication stored at the facility
Corrective action. Ensure children who require medication prescribed by a medical practitioner have access to medication that has not expired.
16020 - CCLR s.53(1)(b) - RECORDS AND REPORTING
The licensee must (b) review the care plan at least once each year with a parent of the child requiring extra support and any person requested by the parent.
Observation. A review of 4 care plans in the Group Child Care (school age) program revealed that 3 out of 4 care plans were not reviewed and updated with parents. The designated supervisor of the Group Child Care (school age) program provided that in one case where the epipen had expired, the child now carries the renewed prescription epipen on their person but there was no updated care plan to provide this change of plan.
Corrective action. Ensure the care plan is reviewed at least once each year with a parent of the child requiring extra support
19260 - CCLR s.58(2)(b) - RECORDS AND REPORTING
A licensee must notify the medical health officer within 24 hours after (a) a child is involved in, or may have been involved in, a reportable incident described in Schedule H while under the care or supervision of the licensee.
Observation. 1. A review of the facility log book of minor accidents, illnesses and minor events contained in the Group Child Care (school age) room, revealed that an incident involving a missing/wandering child had not been reported to licensing within 24 hours after occurrence. A review of information logged did indicate that the child's parents had been notified of the incident. 2. A review of the facility log book in the Group Child Care (30 months to School Age) room revealed that an incident that involved a staff using an emergency hold for a child had not been reported to licensing within 24 hours. The incident did appear to have been reported to the parent of the child. The incident was reported to licensing on the day of this inspection and Licensing will conduct a separate follow up to this incident. This is a repeat contravention
Corrective action. Ensure all incidents described in Schedule H are reported to licensing within 24 hours.
19030 - CCLR s.55(2)(a) - STAFFING
A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (b) character references in respect of the person.
Observation. A review of 5 staff files found that a minimum of two character references were not obtained for one of the employee files .
Corrective action. Ensure all facility staff files are complete
