Observation. Two children's records are incomplete. Both records are missing the child's medical insurance plan number. Children’s records must be maintained with complete and current information to support appropriate response and care in the event of an emergency. Submit by June, 19, 2026, evidence documenting the records are complete. All children's records are missing written consent from a parent to call a medical practitioner or an ambulance. Written consent is required to ensure that emergency medical assistance can be obtained promptly when a parent or emergency contact cannot be reached. Submit by June 19, 2026, evidence documenting the consent has been obtained.

Observation. A change mat is in poor repair with tears in the vinyl covering. Damaged surfaces cannot be effectively cleaned and sanitized, creating a potential health and safety risk.. Submit by June 19, 2026, how this risk has been mitigated.

Observation. The manager confirmed the emergency plan was not practiced at least once in the last year. Implementing the plan at least once each year will ensure employees are prepared in the event of a real emergency. Submit by June 12, 2025, evidence of the plan being practiced.

Observation. Children records are missing other information, such as physical characteristics. This information can be used to readily identify the child in an emergency. Submit by June 12, 2025, evidence that the information has been added to the records.

Observation. It was observed that a piece of wood attached to the base of a sunporch was pulling away from the frame and had a rusty nail protruding from it. The wood and rusty nail pose a tripping hazard and/or a risk to the health and safety of the children. Submit by June 12, 2025, evidence that the wood has been repaired and the nail removed.

Observation. The Facility Licensing Officer reviewed the fire drill practice log and found only one fire drill has been practiced since opening in January of 2024. An employee must practice implementing the fire drill system at least once a month. Submit by the follow up date noted how you are going to ensure fire drills are practiced monthly.
