    Observation. The facility is transporting children in vehicles regularly for off-site field trips. The transportation policy as approved in the application file does not outline the use of a vehicle for transporting children. Licensing is required to be notified immediately when any change in the application file occurs to ensure the health and safety of children in care.     Observation. The cleaning solution used to clean the tables and changetable do not have a drug identification number (DIN).     Observation. It was alleged the Licensee was observed vaping in the presence of the children in care at a local park. The Licensee admitted to vaping at the local park as well as while on walks while supervising the children. Vaping is defined as restricted consumption and is not permitted while children are under the supervision of employees for the health and safety of the children in care. The allegation is substantiated. A response is required by April 23, 2026 that outlines how the health and safety of children in care will be maintained and the risk mitigated. It was discovered in relation to the complaint, the Licensee admitted to vaping in the outdoor play area of the facility while the children are under the supervision of the Licensee. Restricted consumption is not permitted while children are under the supervision of employees on the premise of a community care facility for the health and safety of children in care. A response is required by April 23, 2026 that outlines how the health and safety of children in care will be maintained and the risk mitigated.

    Observation. A person who is not an employee and over the age of 12 was present at the facility at the time of inspection. The person was confirmed by the Licensee to be in attendance at the facility for short periods of time regularly. The Licensee has not obtained a cleared criminal record check for the person.     Observation. One child in care at the time of inspection has additional medical needs that require a care plan. The child does not have a care plan developed on file as required.     Observation. A lighter and DEET bug spray were observed in a drawer in the kitchen within reach and accessible to the children. The lighter and DEET are considered a hazard and may pose a risk of injury in children. Access to hazardous objects and substances are re-occurring contraventions originally found out of compliance on; October 29, 2024, April 24, 2025 and November 26, 2025. A response is required by February 3, 2026 to confirm the risk has been mitigated. Lower kitchen cupboards have child locks to prevent the cupboard from being opened freely by the children. One of the cupboards contains children's toys and games. During the inspection one child was observed to unlock and open the toy cupboard unassisted to put the toys away. The child locks in the kitchen are deemed ineffective if the children in care are able to open the lock without assistance. This leaves the hazardous substances and objects in each cupboard, such as the cleaning supplies, accessible to the children in care and may pose a risk of injury for children. A response is required by February 3, 2026 to confirm the risk has been mitigated and provide evidence for review.

    Observation. Children were observed playing in a separate bedroom of the facility without supervision on multiple occasions during the inspection. It is required that children must be supervised at all times by a qualified person to ensure the health and safety of children in care.     Observation. One child had an Epi-Pen at the facility at the time of the inspection and did not have any corresponding documentation to support the allergy on file. Any medication that may be administered at the facility requires permission from the parent/guardians as well as evidence the Licensee has agreed to administer the medication. A response is required by December 2, 2025 to confirm documentation has been received as required. One child in care requires an Epi-Pen on site in the event of an allergic reaction. The Epi-Pen was disclosed to be kept in a vehicle in the facility driveway. An anaphylactic allergy requires an Epi-Pen to be readily accessible at all times when the child is in care. Having the life saving medication on site and accessible will ensure the health and safety of the child at all times. A response is required by December 2, 2025 to confirm the Epi-Pen is stored as required.

    Observation. A school aged child was in attendance at the time of inspection. It was disclosed that the child had shown symptoms of a communicable illness upon arrival. The Licensee allowed the child to attend the facility knowing the child had symptoms of illness of which prevented the child from attending school. It is the responsibility of the Licensee to operate in a manner that will promote the health, safety and dignity of person's in care. Allowing a child to attend the facility with known symptoms of a communicable illness may affect the health of all children in care.     Observation. It was alleged that a family was not notified that their child was bit, and their child was biting and scratching other children. A review of the minor accident, illness and unexpected events log were found to be completed for incidents involving children. All of the reports involving the child in question did not have evidence of parent notification. The licensee and the staff stated that they inform the parents verbally at the end of the day. Licensees are required to immediately notify a parent or emergency contact if a child becomes ill, injured or is involved in a reportable incident. Based on the information reviewed, and confirmed in conversation with staff, this allegation is substantiated. Submit to Licensing by June 23, 2025, how you will ensure a parent or emergency contact is immediately notified if a child becomes ill, injured, is involved in or may have been involved in a reportable incident. It was alleged that the licensee did not change a child's diapers frequently enough according to the parent's instructions. The facility diapering policy, and the child's record where special instructions regarding diapering was reviewed, however, there was no written instruction from the parent on the frequency of diaper changes. Documentation showing evidence of a minimum of two diaper changes for five days was reviewed. Interviews conducted with families with children in diapers have no concerns with the frequency of changes occurring. The licensee stated that all children are checked hourly and changed as needed. The licensee stated that there was a miscommunication with one family regarding the frequency of diaper changes on the first day of care. When the family requested more changes (2-3 times a day) the licensee agreed and changed their practice. Based on the information received this allegation is unable to substantiate. It was alleged that a child napped in a play pen even though parents requested the child be napped in a bed. The licensee confirmed one family requested their child not to be napped in a play pen. The licensee stated that they followed the family’s instructions and did nap the child in a bed. Based on the information received this allegation is unable to substantiate.

  • C9.1 - Are there plans and actions in place to ensure medications are stored and administered properly?Non-critical

    Observation. It was alleged that the licensee did not apply diaper rash cream containing zinc oxide or administer Tylenol to a child as requested by a family. Interviews conducted revealed that one other family also requested diaper rash cream be applied to their child which the licensee did not comply with. The licensee and staff confirm that diaper rash cream has been provided by parents but kept in children's backpacks and not used. The licensee is not following parent requests to apply or administer medication. Based on the information received this allegation is substantiated. Submit to licensing by June 23, 2025, how you will ensure parent's medication requests are complied with.

    Observation. Children's files are incomplete. A sampling of files reviewed are missing: medical insurance plan number, status of immunization, start date of care and a name and telephone number of a medical practitioner. This is a re-occurring contravention originally found out of compliance on October 29, 2024. A Licensee must keep current records for each child that includes all documentation as required to be referred to in the event of an emergency. A response is required by May 5, 2025 to confirm all children's files are complete and provide evidence of all completed files. The daily attendance record is incomplete and missing time of arrival and departures for more than one day of children in care. The Licensee is required to keep a record of attendance that indicates whether a child is absent or, if the child is present, the time of arrival and departure. It is important to have an up to date account of who is in care at any given time in the event of an emergency. A response is required by May 5, 2025 to confirm how attendance will be kept up to date. Two children's files reviewed are missing written consent to call a medical practitioner or ambulance. This is a re-occurring contravention originally found out of compliance on October 29, 2024. All children in the care of the Licensee must have written consent to call a medical practitioner or ambulance on file in the event of an emergency. A response is required by May 5, 2025 to confirm consents have been obtained as required.

    Observation. It was disclosed that children are not regularly washing hands prior to eating snacks or meals at the facility. This is a re-occurring contravention originally found out of compliance on October 29, 2024. A program to instruct children on proper hand washing before eating is required to practice the rules of health and hygiene and to prevent the spread of illness. A response is required by May 5, 2025 that outlines how handwashing will be implemented as required.

    Observation. Two children were observed tripping and falling over a carpet in the play area. One child hit their head on the floor when they fell down. The edges of the carpet are a tripping hazard and increase the risk of injury for children in care. A response is required by May 1, 2025 to identify how the hazard was addressed. One child was observed sleeping on the couch in a slumped over sitting position when the Licensing Officer arrived for inspection. Other children were observed sitting on the couch while the child was sleeping. The child is at risk of rolling off the couch when sleeping as observed. Furniture or equipment for sleeping is required to be used by one child at a time and is clean, comfortable and appropriate for the size, age and development of the children intended to use it. A response is required by May 1, 2025 to identify how children will be offered appropriate equipment for sleeping. A gate was observed in the outdoor play area that was propped up against the fence with sharp metal hinges within the children's reach. The sharp edges of the hinges pose a risk of injury in children. A Licensee is required to ensure that children do not have access to any object or substance that may be hazardous to the health or safety of a child. A response is required by May 1, 2025 to identify how the hazard has been addressed. A climbing structure that includes a slide and a swing set has been set up in the outdoor play area of the facility. The structure does not have the required 6 foot clearance around it. A six foot clearance is required to avoid the risk of injury of children playing on or around the equipment. A response is required by May 1, 2025 to address the clearance of the climbing structure. The climbing structure in the outdoor play area has been installed directly onto the grass without any protective surfacing under or around the structure. Protective surfacing is required to reduce the risk of injury in children playing on the equipment. A response is required by May 1, 2025 to address the climbing structure and the surfacing required.

  • C8.2 - Is each child provided a comprehensive and coordinated program of developmentally appropriate activities that encourages physical, intellectual, language, emotional, and social development?Non-critical

    Observation. The children were watching television when the Licensing Officer arrived for inspection. It was disclosed by the Licensee that the children had been watching TV for approximately 45 minutes prior to Licensing arrival. It is the responsibility of the Licensee to ensure any screen time offered to the children does not exceed 30 minutes per day. A response is required by May 2, 2025 that outlines how screen time will be addressed.

    Observation. Upon Licensing arrival a television show was being offered to the children in care. One of the children in attendance is under two years old. As per the Director of Licensing Standards of Practice - Active Play: Ensure no screen time is offered to children under 2 years of age.     Observation. It was disclosed that children are eating lunch and meals on the couch in the home. A surface that can be sanitized prior to eating and after eating is finished is required to be used for children to eat to maintain the health and hygiene of the children in care. Outline the system developed to provide a surface for children to eat that can be sanitized as required. A sharp set of plyers is in a drawer in the bathroom within reach of the children. The plyers were moved out of reach of the children during inspection. Sharp objects that may be hazardous to the health and safety of a child must be kept out of reach at all times.
