A licensee must keep, for each child, a record showing the following information: (d) name and telephone number of a parent, medical practitioner and emergency contact.
Observation. Doctor's information was not available on one child's file. The Manager said that the family goes to a walk-in clinic and Licensing explained that this information can be included in exchange for a Dr's contact info.
Corrective action. Please ensure that children's files are complete.

A licensee must keep, for each child, a record showing the following information: (a) name, sex, date of birth, medical insurance plan number and immunization status.
Observation. All children's files were reviewed and some files were missing care card numbers. Some of this information was found on the individual child's immunization file. Discussion was had with the Manager regarding including this information on the registration form.
Corrective action. Please ensure children's files are complete.

A licensee must keep, for each child, a record showing the following information: (c) daily attendance record, indicating for each day whether the child is absent or, if the child is present, the time of arrival and departure.
Observation. No attendance records were available since April 2020. The Licensee stated that parents have not been signing their children in and out since the start of COVID-19 so the Licensee did not think she could sign the children in/out herself.
Corrective action. Please ensure that attendance is recorded everyday with the arrival and departure time.
